Learn more Permanent Change of Station (PCS): Not Authorized Financial Disclosure Report: Not required The incumbent is assigned as the Hospice Palliative Care (HPC) Social Work Program Coordinator at Sierra Nevada Health Care System under HPC program in Geriatrics and Extended Care. HPC Services are designed to provide interdisciplinary supportive care to Veterans facing a life-limiting illness and their family members. The Hospice and Palliative Care team work to ensure that Veterans are as comfortable as possible, addressing physical, emotional, psychological, and spiritual pain. Supportive counseling addressing grief and loss are an integral part of the care for Veterans and their families. The HPC program is designed to provide specialized knowledge of practice related to end-of-life care and identify Veterans in need of palliative services. Duties of the position include, but are not limited to: Responsible for developing and maintaining a system to identify hospice and palliative eligible Veterans served by the facility and community, thus ensuring access to services. Responsible for managing consults per HPC Program Standard Operating Procedure (SOP). Contacts the Veteran and family to offer supportive counseling and education and prepares packets for referrals to community hospice agencies once the medical provider has certified hospice eligibility. Ensures that packets are submitted in a timely manner. Develops and implements a plan for outreach to extend services to Veterans who may benefit from care, but who do not presently use VA for their healthcare needs. Holds a high degree of responsibility for program operation, develops protocol, standards for referrals and will monitor referrals to community hospice agencies. Incumbent serves as point of contact/liaison for inquiries regarding the Hospice Palliative Program. Responsible for tracking and management of budget and fund control for the hospice program. Completes psychosocial assessments for seriously ill patients and their families and develops a treatment plan for the veteran in collaboration with the interdisciplinary Hospice and Palliative Care team, the veteran, family members and/or decision maker. Provides independent and expert clinical psychosocial and case management services in a specialized area of practice to Veterans who tend to have serious and severe crises, may lack any familial and community support, may be poor self-monitors, may frequently fail to comply with instructions and treatment, or may have major deficits in coping skills and require continuing professional psychological support. Work independently with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, financial, legal and psychosocial problems. Responsible for developing the treatment plan and setting achievable treatment goals with the veteran/family / decision maker in collaboration with the HPC interdisciplinary treatment team members. Collaborate with other service providers in reassessing the veteran's needs for non-institutional, institutional services/programs and entitlements. Provides diverse clinical services to Veterans and family members/significant others in order to ensure the successful implementation of treatment plans designed to maximize the attainment of their full potential. Responsible for educating the veteran and/or caregiver of the available services and assisting them in establishing the appropriate referrals based on the veteran's preference or that of his surrogate decision-maker. Responsible for using the current social work resource file of VA and community social service programs and enhancing the content to the benefit of veterans. Provide education related to VA and community resources, benefits, Advance Directives/Living Wills, Advance Care Planning and Goals of Care Conversations and will refer veterans/families to the appropriate interdisciplinary team member for identified health education needs. Conducts outreach to community-based Hospice Agencies, Group Homes, Assisted Living facilities, Nursing Homes, etc. who provide care to Veterans on behalf of VA, or have an interest in doing so, encouraging formal and informal partnerships with VA. Refer the Veteran to Integrated Behavioral Health Clinic (IBHC) or Mental Health services as needed. Provide supervision for MSW trainees as well as supervision to staff members in the process of seeking independent or advanced practice licensure. Must have and maintain a valid unrestricted driver's license. Other duties as assigned by management.

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. ? 7403(f). GRADE DETERMINATIONS. In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts.] References: VA Handbook 5005/120 Part II Appendix G39, Dated September 10, 2019. The full performance level of this vacancy is GS-12. Physical Requirements: Work is primarily sedentary. May require prolonged standing, moderate walking, and manual dexterity. Requires carrying light items such as books, files, or laptop computer. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
