This full-time position is located at the Alaska VA Healthcare System, an ambulatory care facility located in Anchorage, Alaska with a Homeless Domiciliary and Community Based Outpatient Clinics in Fairbanks, Wasilla, Soldotna, Homer and Juneau, Alaska.

Duties

Responsibilities and duties are but not limited to the following: Consistently make and implement sound and reasonable patient care and consulting decisions and provide leadership within the pain care team. Provide and assisting with the coordination of care for chronic pain patients enrolled with the Comprehensive Pain Management Clinic. Provides consultation service locally and regionally for providers working with complex chronic pain care, and provides education to patients and to providers regarding safe and sustainable chronic pain care and effective functional restoration of patients suffering with chronic pain. Completes detailed history and physical exams, as well as health inventories, orders and interprets diagnostic studies and prescribes appropriate medication and other treatment modalities. Develops, evaluates and manages medical treatment plans intended to achieve defined outcomes that improve the patient's quality of life as part of a coordinated multidisciplinary treatment team focused on safe and sustainable chronic pain care and functional restoration of patients living with chronic pain. Demonstrates expertise in diagnosing and treating complex chronic pain problems. Consistently makes and implements sound and reasonable decisions and has responsibility for patient outcomes. Provides care management that includes timely, thorough, and efficient documentation & coordination. Orders, performs, reviews, analyzes and follows up on appropriate diagnostic/laboratory test results, including any registries necessary to monitor and support the patient's medication therapy, and communica Orders, performs, reviews, analyzes and follows up on appropriate diagnostic/laboratory test results, including any registries necessary to monitor and support the patient's medication therapy, and communicates findings to patients and other providers, including additional surveillance and/or testing requirements necessary as per VA policy. Initiates consults as appropriate, follows the treatment plan of care, coordinates and discusses plan with other care providers and specialists as needed. Establishes and maintains contact with other care providers, specialists, pharmacy and ancillary staff as necessary within the facility and the private sector. Review non-VA records and updates CPRS and/or other electronic health records with outside records. Provides consultation service locally and regionally (VISN) for providers working with complex chronic pain care. Performs suicide and opioid risk screenings and assessments. Treats outpatients with chronic pain and provides opioid reduction, harm reduction, and opioid safety/Medication Assisted Treatment (MAT) services as needed. Initiates, modifies, or discontinues medication/opioid therapy and serve as a consultant for intensive medication therapy management services. Provides patient care via multiple modalities, including in person, video, and telephone visits on an individual or group basis, as well as electronic consults for referred patients for comprehensive pain management. Performs interventional pain medicine strategies as part of a balanced multidisciplinary treatment plan. Interacts closely with other clinicians, including physicians, PAs and ARNPs, psychologists and physical therapists in the design of the veteran's pain regimen to achieve optimization of rational and cost-effective pain management. Serves as a clinic level patient advocate and addresses patient complaints and concerns as they arise. Other clinical duties as assigned. Qualifications

To qualify for this position, you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement. Applicants pending the completion of training or license requirements may be referred and tentatively selected but may not be hired until all requirements are met. Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR [(2) Those approved by the American Osteopathic Association (AOA),OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences. Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs. Proficiency in spoken and written English. Additional Requirement: Certification: Must be certified by the American Board of Anesthesiology, or board eligible. Fellowship: Completed a Pain Medicine Fellowship. Preferred Experience: - Board certification in Pain Medicine is strongly preferred. - Additional Board certification in Addiction Medicine is strongly preferred. - Strongly prefer experience with buprenorphine and other Medication Assisted Treatment modalities for opioid use disorder. - Strongly prefer at least 5 years of clinical experience as a Pain Medicine Specialist after graduation from an ACGME-accredited pain fellowship. - Strongly prefer at least 5 years of experience in a multidisciplinary pain clinic setting. - Quality Improvement experience preferred Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-2 Physician Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Heavy lifting, 45 pounds and over, Light lifting, under 15 pounds, Light carrying, under 15 pounds, Use of fingers.

Education

Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine. The degree must have been obtained from an institution whose accreditation was in place for the year in which the course of study was completed. Approved schools are:
  1. Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted, or
  2. Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation of the American Osteopathic Association for the year in which the degree was granted.
  3. For foreign medical graduates not covered in (1) or (2) above, facility officials must verify with the Educational Commission for Foreign Medical Graduates (ECFMG) that the applicant has met requirements for certification, and must obtain a copy of the ECFMG certificate, if claimed by the applicant. [If the applicant does not claim an ECFMG certificate, facility officials must still confirm that the medical school meets (or met) ECFMG eligibility requirements for the year the candidate graduated.]
NOTE: The Under Secretary of Health or designee in the VHA Central Office may approve the appointment under authority of 38 U.S.C. 7405 of a physician graduate of a school of medicine not covered above if the candidate is to be assigned to a research, academic, or administrative position with no patient care responsibilities. The appointment will be made only in exceptional circumstances where the candidate's credentials clearly demonstrate high professional attainment or expertise in the specialty area.
