This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

The Clinical Pharmacy Specialist functions as a physician extender with a comprehensive Scope of Practice which includes prescriptive authority, monitoring drug therapy, ordering of laboratory test, and other specified clinical functions. The incumbent will be responsible for managing a pharmacotherapy clinic and Primary Care/Geri PACT to provide care of veterans with a variety of chronic health disorders (Hypertension, Diabetes, Hyperlipidemia, COPD, CHF, Smoking Cessation, etc.) The professional and administrative management of the Anticoagulation Therapy Programs services provided by the primary Pharmacy Services CLINICAL: Obtain medication history from each new patient and summarize the significant findings, both documented in the medical record and reported to the responsible physician when appropriate. Identify all drug induced problems that may be affecting the patient or contributing to a need for hospitalization. This would include drug side effects, allergic reactions, toxicities, and improper dosage regimens, non-compliance, etc. Make recommendations to the responsible physician for optimal management of drug induced problems either verbally or in writing. Evaluate the risk benefit ratios of various therapeutic alternatives, develop a therapeutic plan utilizing the most effectives, least toxic, and most economical pharmacological treatment modalities available, and communicate these plans to the patients physician. Manage the medication programs of patients by electronically ordering appropriate medications, diagnostic studies, etc., in consultation with the responsible physician, within their scope of practice. Select appropriate objective and subjective parameters necessary to monitor the pharmacologic and toxic effects of the drugs being administered. Analyze the patients' medication regimen for drug-drug interactions and determine the potential and significance of each interaction. Analyze the patient medication regimen and laboratory test data for drug interference with laboratory tests. Develop rational alternative therapeutic treatment plans in cases where the initial selected plan fails to achieve the desired therapeutic result, problem occur with drug toxicity or other complications and communicate this information to the patient's physician. When assessing or providing patient care, the employee will evaluate the impact of the age of the patient as it relates to ability to understand directions, provide objective self-assessment or any other medical milieu appropriate to the overall treatment plan. May need to assist with anticoagulation management, monitors, and adjust INR 's in relation to overall patient status, evaluate current medications and regards to drug/drug, drug/food interactions, refer patients to other treat treating specialties, UCU/ER, clinics as appropriate, and assure appropriate follow- up as necessary. EDUCATION/RESEACH: Precepts the Primary Care/Geri PACT rotation offered to students from the University of Kentucky and pharmacy residents in this institution's Pharmacy Residency Programs. Attend and actively participates in all clinical pharmacy activities, i.e., staff development, residency conferences, journal club. Contribute regularly to the Pharmacy Service Newsletter, a periodic newsletter prepared by the section of clinical pharmacy and distribute it to all physicians, pharmacist, nurses, and dietitians. This newsletter is also routinely distributed to other Vas in the Veterans Integrated Service Network (VISN) VISN 9 Network. It provides scientific and technical articles, as well as general drug information and pharmacy policy Serve as the consultant and resource person for the Drug Information Center for all questions relating to the elderly population. Prepare and present educational lectures to a variety of disciplines in the area of Pharmacy Care topics Assist in lectures/teaching responsibilities of VA clinic pharmacy service for the UK College of pharmacy, as assigned Participate in the design and implementation of research protocols, which includes research and data, written proposal, data collections, etc. ADMINISTRATIVE/SUPERVISORY: Attend, participates, and contributes as an active member on various Health Systems or VISN committees as assigned Co-Manage assignments of Clinical Pharmacy Specialists to all Primary Care Teams and duties in order to assure continuous and consistent coverage of the clinic Monitors patient scheduling to assure appropriate patient follow-up and time to next clinic availability Work Schedule: Monday - Friday, 8:00am - 4:30pm Telework: Ad-hoc telework may be available. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. OR, Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Professional practice means paid/non-paid employment as a professional or unlicensed graduate pharmacist as defined by the appropriate licensing board. 1. Experience: In addition to the basic requirements, applicants must have 1 year of experience equivalent to the next lower grade level (GS-12). AND; 2. KSAs: Pharmacists assigned to this position must demonstrate the following KSAs: a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. Advanced skill in monitoring and assessing the outcome of drug therapy.
