About the Position: This position is with the Defense Health Agency, Naval Hospital, Directorate of Medicine, Emergency Medicine, Agana Height, Guam.

Duties
Provides comprehensive, holistic, and compassionate medical management services to beneficiaries with catastrophic, serious, chronic, and/or high-risk illnesses or conditions and/ or high-utilizer beneficiaries.
Assesses the physical, emotional, social, economic and spiritual needs of the patient, and when feasible those of his/or her family and caregiver.
Facilitates and coordinates patient care activities to include required tests, procedures, treatment, discharge planning and community referrals.
Ensures that the patient and their family are screened for social service needs, home care, and other community care needs and that referrals are coordinated and made as necessary and appropriate by the responsible discipline.
Designs, implements and supports optimal resource utilization, cost effectiveness and ongoing quality improvement of the Medical management program.
Provides the patient and their family or caregiver with the knowledge and skills necessary to promote self-determination, self-management and implementation of the established plan.
Requirements
- Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation.
- This position has a mandatory seasonal influenza vaccination requirement and is therefore subject to annual seasonal influenza vaccinations.
- Must have active, current registration as a professional nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
- In accordance with BUMEDINST 1500.15F employees assigned to or subject to being assigned to duties providing direct patient care, either diagnostic or therapeutic must possess and maintain Basic Life Support (BLS) certification.
- IAW BUMEDINST 6230.15, susceptible or occupationally exposed health care employees who have direct contact with patients will receive appropriate immunization against communicable diseases unless a current immunization.
- This policy applies to all health care settings, regardless of age or sex of the health care employee.
- Employees who have contact with or potential exposure to human blood or blood products (patient care, laboratory, or other health care settings) are provided hepatitis B virus vaccine IAW the local blood borne pathogen exposure- control plan.
- IAW DoD Instruction 1402.05, existing and newly hired personnel involved in the delivery of healthcare to children under the age of eighteen-years-old on a frequent

- This position is a Testing Designated Position (TDP) subject to applicant testing and random drug testing.
- The incumbent may be required to work on any day or shift, including night, weekends, holidays, or on an on call basis, necessary to meet mission requirements.
- This position requires a pre-employment physical examination.
- This position requires the incumbent be able to obtain and maintain a determination of eligibility for a Secret security clearance or access for the duration of employment.
- This position has a requirement to lift up to 20 lbs.
- This position requires shift work to provide coverage on evenings, weekends, holidays, or special situations. Employee shifts are rotated on a regularly scheduled basis.
- This position requires the incumbent to work on an intermittent or unscheduled, on-call basis.

Basic Requirement for Nurse (CASE MANAGER):
Degree: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Licensure: A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination.
Foreign Degree: A degree from a Foreign Nursing School. Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Exemption: You are exempt from the basic requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before. Applicants claiming this exemption must meet the requirement below. Note: You will be required to provide appropriate documentation to the respective Human Resources Office to validate your status.
In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:
Education and Experience: Possession diploma, associate degree, or bachelor's degree in Nursing from a professional nursing program and possession of at least one full year of professional nursing experience (equivalent to the GS-09 grade level or above) which includes coordinating patient care, educating patients or staff, and assisting with developing patient care plans. Education: Successful completion of two and a half years of progressively higher level graduate education leading towards a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.

FOREIGN EDUCATION: If you are using education completed in foreign colleges or universities to meet the qualification requirements, you must show the education credentials have been evaluated by a private organization that specializes in interpretation of foreign education programs and such education has been deemed equivalent to that gained in an accredited U.S. education program; or full credit has been given for the courses at a U.S. accredited college or university.
