This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

The Home Based Primary Care (HBPC) NP manages a panel of adults patients throughout the continuum of primary care; acute, chronic and preventative care for a panel of patients with complex medical and social needs in the home. Works independently in the home and collaboratively with the entire team to provide holistic quality care to the Veteran. Is part of an interdisciplinary team consisting of MD, RNCM, SW, RD, OT, pharmacist and Psychologist. The Home Based Primary Care (HBPC) Nurse Practitioner (NP) will work with an interdisciplinary team consisting of MD, RNCM, SW, RD, OT, pharmacist and Psychologist. The NP will have a maximum of 3 RNCM with a panel size of +/- 90 Veterans and will be responsible for the delivery of care to these Veterans in the Veterans home. NP may have their own panel of patients of +/- 10 patients. The NP's will provide professional care via face to face home visits, telephone and electronic contact. Care involves medical services that include patients having complex, chronic disease states with complications requiring treatment in the home and triaging to higher level of care as needed. The NP initiates and coordinates referrals to other health care providers, services and agencies. The NP will examine patients and establish medical diagnoses by client history, physical examination, and other criteria. Order, perform and interpret laboratory, radiographic and other diagnostic tests. Identify, develop, implement and evaluate a plan of care for a patient to promote, maintain and restore health and write orders for such. Provide intake to establish care and focused care visits for patients. Identify patients within their panels who are beyond the scope of their education and responsibilities and arrange appropriate transfer and/or support with their collaborating physicians. Provide for patient coverage in cases of the absence of either the NP or the collaborating physician. Meet at least weekly as a team to discuss specific patients, direct their care, and formulate care plans. The NP must Possess a valid unrestricted driver's license. Is expected to drive to veteran's place of residence in order to provide skilled services when addressing needs for the HBPC Program clients. Consistently demonstrates the skills and competencies necessary to function autonomously within the scope of their professional practice with strength in critical thinking skills, best practices, and evidenced based practice. Must possess positive interpersonal skills that facilitate good communication and care coordination between the veteran, caregiver and the HBPC. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Current certification as a nurse practitioner from Current Certification as a Nurse Practitioner Certification from American Nurse Credentialing Center (AACN) or American Academy Nurse Practitioner Certifying Board (AANP) possess a Masters degree from a program accredited by National League for Nursing Education Accreditation. Preferred Experience and Certifications: Family Nurse Practitioner (FNP) Certification, Adult-Gerontology Nurse Practitioner or Acute Care Nursing Practitioner (ACNP) with Emphases in Adult Gerontology. Home Care Experience & Geriatric experience: 3 years successful NP experience, ability to work independently and with interdisciplinary team, excellent communicator and team player. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse II - A BSN with approximately 2-3 years of nursing practice/experience a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Heavy lifting (45 lbs and over); Heavy carrying (45 lbs and over); Straight pulling and pushing; reaching above shoulder; use of fingers; both hands required; walking and standing up to 8 hours; repeated bending; Ability for rapid mental and muscular coordination simultaneously; hearing aid permitted.
