CTVHCS is in the process of making a cultural change, shifting from a disease-focused approach to a health and wellbeing approach, in order to implement the Whole Health System of Care. This is a VA-wide initiative to provide a comprehensive and systematic approach to improve healthcare delivery at all stages of Veteran care. This entails moving from an episodic, "siloed" approach to an interdisciplinary, holistic approach, emphasizing self-care in the larger context of well-being. All staff in this facility will be part of this process, shifting the focus from the question, "What's the matter with you?" to the question, "What matters to you, and how can we help you live your best life?" This strategy also involves integrating a full range of conventional approaches with Complementary and Integrative Health (CIH) approaches in order to optimize lifelong health and well-being, as defined by the Veteran, rather than narrowly focusing on disease management.
You will collaborate with multiple disciplines in the assessment, planning, organization, implementation, and evaluation of an effective program including orientation, continuing education, and in-services. Establish effective relationships with all Medical Center personnel and serves as a liaison. You are a role model in demonstrating excellence in practice, serves as a clinical mentor/preceptor for clinical and administrative staff, consults and collaborates in patient care and research, and educates in both formal and informal situations. Demonstrate leadership in delivering and improving holistic care through collaborative strategies with others. Initiate and maintain databases, prepare reports, and work collaboratively with others to assess variances and project needs in training processes. Facilitate recommendations for systems and/or resource changes that will serve to improve the work force and resource utilization. Coordinate and conduct staff education including clinical orientation, mandatory reviews, staff development, skills assessments, and continuing education opportunities. Demonstrate expert written and verbal communication skills. Conduct review, evaluation, and revision of educational programs annually and as needed. Conduct an annual needs assessment for clinical staff in assigned areas and analyze learner needs against strategic plans and initiatives to determine education to be provided throughout the year. Maintain evidence-based clinical practice knowledge and current knowledge of patient care delivery models and work design in areas including but not limited to extended care, acute medical care, specialty, primary care, outpatient, and mental health.
