This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

This position is in the James J. Peters VA Medical Center in the Bronx, New York and provides support to the Pharmacy Program. The James J. Peters VAMC is a tertiary care facility classified as a Clinical Referral Level 1 Facility. It is a teaching hospital, providing a full range of patient care services, with state-of-the-art technology as well as education and research. Comprehensive health care is provided through primary care, tertiary care, and long-term care in areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, geriatrics, and extended care. James J. Peters VAMC has 311 authorized hospital beds and 120 nursing home beds and operates several regional referral points including Spinal Cord Injury (SCI) and VISN referrals for Medical/Surgical subspecialties. For example, the James J. Peters VAMC SCI Patient Care Center is the referral point for VISN's 2, 3 and 4 as well as Department of Defense (DOD) new SCI patients. The James J. Peters VAMC provides a broad range of inpatient and outpatient health care services and has three Community Based Outpatient Clinics (CBOC's) serving Bronx, Northern Queens and Westchester counties. We also serve VA Hudson Valley Health Care System patients with acute care needs and referrals to subspecialty clinics. The Clinical Pharmacist works with a primary emphasis on inpatient care, outpatient care, and pharmacy-based clinical services. These duties are expected to be carried out in an independent manner. The clinical pharmacist is a licensed professional with knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics. Some duties include but are not limited to: CLINICAL 1. The Clinical pharmacist provides appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers as appropriate. 2. Provides patient-specific therapeutic drug monitoring and communicates relevant findings and/or recommendations to other health care providers in charge of the patient both orally and in writing. Monitoring will include prospective review and intervention in: 3. Therapeutic appropriateness of a patient's drug regimen 4. Therapeutic duplication in the patient's drug regimen 5. Appropriateness of the route and method of administration DISPENSING 1. Reviews all medication orders for appropriateness, drug selection, dosage, route of administration and the amount, prior to dispensing. 2. Assures that all prescriptions and actions meet proper eligibility criteria on a patient specific basis and that the National Formulary and Non-Formulary procedures are followed. This includes adherence to all national, VISN and local criteria or use treatment algorithms. 3. Ensures all medication orders are entered into the electronic patient medical record. 4. Fills all orders in a timely fashion; review of the patient's allergy history, drug-drug, drug-nutrient, drug-laboratory and drug-disease state interactions and assessment for clinical relevance should be performed and dealt with appropriately. 5. Reviews medication profiles and monitors for compliance or potential abuse; problems encountered shall be resolved. EDUCATION 1. Provides education to patients, care givers, and health care providers regarding medication use. 2. Maintains basic knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics in the area of practice 3. Assumes an active role in the overall pharmacy-training program consisting of students, externs, interns, residents, and professional and non-professional staff members. Conducts on-the-job training and provides guidance through designated programs. 4. Keeps abreast of new developments in pharmacy practice and drug therapy by reading pharmacy and medical literature and participating in continuing education programs. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). (This is required to supply if you have foreign education) Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification. Exception Non-licensed pharmacists who otherwise meet the eligibility requirements may be given a temporary appointment at the entry level as a Graduate Pharmacist under the authority of VA HANDBOOK 5005/55 JUNE 7, 2012 PART II APPENDIX G15 II-G15-2 38 U.S.C. 7405(c)(2)(B). The appointing official may waive the requirement of licensure for a period not to exceed 2 years for a pharmacist that provides care under the supervision of a licensed pharmacist. For grade levels above the GS-11, the candidate must be licensed. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d) May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Creditable Experience Knowledge of Professional Pharmacy Practices. To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Professional practice means paid/non-paid employment as a professional or unlicensed graduate pharmacist as defined by the appropriate licensing board. Residency and Fellowship Training. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for-year basis. The pharmacy residency program must be accredited by the American Society of Health-System Pharmacists (ASHP). A fellowship program that is not accredited by the American College of Clinical Pharmacy (ACCP) will need to have comparable standards for experience to be creditable (Professional Standards Board refers to the Deputy Chief Consultant for Professional Practice for the determination). Quality of Experience. Qualifying experience must be at a level comparable to pharmacy experience at the next lower level. Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist (under supervision) and subsequently passed the appropriate licensure examination. Part-time Experience. Part-time experience as a professional pharmacist is credited according to its relationship to the full-time workweek. For example, a pharmacist employed 20 hours a week, or on a 1/2-time basis, would receive 1 full-time workweek of credit for each 2 weeks of service. Grade Determinations. In addition to the basic requirements for employment above, the following criteria must be met when determining the grade of candidates. GS-12 Clinical Pharmacist (Full Performance Level) Experience or Education: Candidates must meet one of the following: A. 1 year of experience equivalent to the next lower grade level, or B. Completion of an ACPE-accredited Pharm.D. program. Assignment. A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution. Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs): 1. Knowledge of professional pharmacy practice. 2. Ability to communicate orally and in writing to both patients and health care staff. 3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. 4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Residency Experience or 3yrs Pharmacist Experience Physical Requirements: Work requires long periods of sitting, standing and walking, bending, twisting, and carrying items up to 25lbs. Fine motor skills for the manipulation of products are also required. Work Environment: Work is performed in inpatient units, clinic settings, and within an outpatient/ inpatient Pharmacy setting. The position requires patient interaction which requires universal precautions and other normal safety precautions. The incumbent wears personal protective equipment (PPE) when in contact with patients with a contagious disease or when in contact with contaminated drainage.
