The Finger Lakes Health Care System is currently recruiting for a Social Worker (Program Coordinator) for the Spinal Cord Injury (SCI) Primary Care Team Program and the Preservation-Amputation Care and Treatment (PACT) Program. The Social Worker will develop policies and procedures for program operation and prepare reports for facility, VISN, and national use. The incumbent plans, organizes and directs administrative, operational, patient and personnel activities for the program services.

Major Duties:
Develops a system to identify SCI patients who may be served by the facility and implements outreach plans to extend services to underserved SCI veterans.
Administers information and analytical systems to evaluate and enhance the quality of service provided to patients. Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction.
Accountable for program effectiveness and is responsible for the management of the nationwide SCI Registry to include the entering, editing, compiling and analyzing and updating of registry data.
Performs annual assessments and case reviews to ensure that SCI registrants continue to engage in active treatment. Develops and implements realistic and achievable social work treatment plans, as part of an interdisciplinary treatment effort, that reflect an ability to effectively utilize a broad range of treatment modalities.
Promotes and coordinates annual comprehensive preventive health evaluations for SCI vets at the SCI Center or SCI Support Clinic.
Provides consultation to other staff, patients and their families regarding the appropriate level of care within the VA.
Recommends and initiates prosthetic requests for adaptive equipment for SCI veterans.
Possesses an in-depth knowledge of the programs coordinated and has demonstrated knowledge and ability to write policies, procedures and/or practice guidelines for the program including program memoranda.
Explores and analyzes long term care placement options in the community as well as the provision of in-home supports that enable continued functional independence among SCI registrants. Identifies unmet needs of these populations and designs/proposes modifications to service delivery patterns to facility administration as warranted.
Develops, implements, and evaluates clinical guidelines and protocols to establish appropriate utilization of services.
Independently conducts psychosocial assessments. Develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team.
Provides individual, family and group counseling aimed at helping veterans and their families find practical solutions to complex problems. Maintains therapeutic relationships with SCI veterans and their families.
Addresses veterans' psychological, social, and vocational needs and effectively coordinates these and other medical services.
Makes home visits to assess the home environment when appropriate in order to ensure that services provided will enable veteran to live safely and independently. Collaborates closely with colleagues at Community Based Outpatient Clinics in addressing outpatient needs of veterans enrolled at this facility and other programs. Occasionally meets veterans at these locations in order to further patient care needs.
Provides advanced directive (Living Will and Durable Power of Attorney for Health Care) information and education to veterans and their families and assists them in preparing and processing such documents as requested.
Establishes and maintains ongoing educational efforts for patients and staff to facilitate understanding of issues pertaining to care of veterans with spinal cord injury.
Responsible for ensuring that the facility SCI programs remain interdisciplinary and fully integrated with the inpatient and outpatient medical programs. Establishes collaborative relationships with physicians, nurses and other disciplines to foster their involvement in the care of veterans with SCI.
Conducts regularly scheduled meetings of the SCI Primary Care Team in order to monitor and improve the quality of services provided to SCI veterans at this medical facility. Formulates agendas and carries out plans for addressing actionable items and regularly reports to the Executive Committee of the Medical Staff on the operations of programs.
Serves as the point of contact for all inquiries regarding programs with stakeholders such as veterans, families, community hospitals and other treatment facilities, veteran's service organizations and advocacy groups. Represents medical facility at VISN and national conferences pertaining to service delivery for SCI veterans.
Performs a wide range of services and provides technical assistance to State, local and private agencies. Performs program analysis, development, review, research, and contact work with various agencies (public, private, state, et cetera).
Performs other related duties as assigned.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: GS-12
Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs:
(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
(b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
(c) Ability to supervise multidisciplinary staff assigned to the program.
(d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
(e) Ability to provide training, orientation, and guidance within clinical practice.
