The purpose of this position is to serve as a Program Support Assistant within the Home Based Primary Care section of the Geriatrics and Extended Care Service. The position reports to the HBPC director and performs various administrative and clerical duties in support of the HBPC program.
Duties
Performs work involving the collection, compilation, and/or tracking of data and statistical information in support of the service's operation.
Reviews and uses reports from various systems to ensure that patient information is accurately updated and entered into the HBPC package.
Reviews error reports for accurate encounter documentation and transmission of data to Austin.
Accepts and dispositions any telephone calls from patients, caregivers, HBPC staff in the field, other VA staff, and/or community agency staff, regarding program inquiries, patient care concerns, etc.
Handles all vendor scheduling for in-services HBPC staff.
Develops and maintains HBPC reports using Excel, PowerPoint, and Software Computer Packages to create and compile weekly, monthly, quarterly, and annual reports.
Interacts daily with HBPC staff, patients and their families, other VA staff, and persons outside the VAMC ranging from other government agencies to the public.
Explains services provided by HBPC and other programs within the VAMC to inquiring individuals.
Refers to and coordinates with other VAMC programs when appropriate.
Answers the telephone, arranges and compiles agendas, attends and records meeting proceedings.
Keeps schedules and timelines of needed visits from various parties (Coordinator, Fire and Safety Officer, etc.), as well as ensures that required training sessions are performed and documented in a timely manner.
Processes incoming and outgoing materials, such as correspondence, reports, memoranda, and other forms
of written communication.
Coordinates meetings and/or schedules conferences.
Maintains excel calendar of all HBPC veterans for quarterly treatment team reviews and creates weekly team meeting agendas accordingly.
Provides clerical services such as maintaining multiple calendars and recording appointments, arranging for meeting and small conference administrative details, or following up on action items with appropriate staff members independently.
Attends meetings, notes commitments made, informs staff of commitments and follows up.
Performs other related duties as assigned.
Work Schedule: Monday through Friday, 8:00 am - 4:30 pm

GS-06 grade level: One year of specialized experience (equivalent to the GS-05 grade level in the federal service); experience that equipped the applicant with the particular knowledge, skills and abilities (KSA's) and other characteristics to perform successfully the duties of the position, and that is typically in or related to the work of the position to be filled, in the normal line of progression for the occupation in the organization. Specialized experience includes: Comprehensive knowledge of rules, regulations, and other guidelines relating to completing assignments for the assigned area. Knowledge of HBPC, MFH, and its relationship to other Home and Community Based Care Programs within the VA. Knowledge of issues in outpatient processing, eligibility, admissions, software applications of all administrative and ancillary services, and legal aspects of processing consents forms outpatient processing, eligibility, admissions, software applications of all administrative and ancillary services, and legal aspects of processing consents forms, Advance Directives, guardianship, incompetence, Powers of Attorney, Privacy Act, and Freedom of Information Act. Knowledge of various VA directives, Medical Center Memorandums, procedures, and publications pertinent to home patient care (i.e., monetary benefits, types of discharge, eligibility for care, etc.) and the priorities associated with them. Knowledge of VA Policies, regulations, and philosophy, to assure all actions are in conformance with all statutory and regulatory requirements. Knowledge and ability to apply patient privacy regulations and release of information procedures. Knowledge of age or disease-related conditions as they affect communication, and the ability to adjust communication style accordingly. Knowledge sufficient to recognize which information is or is not relevant to the problem at hand. Skill in operation and routine maintenance of facsimile and copier machines, as well as personal computers and printers. Ability to plan effectively, set priorities, have a high degree of skill in oral and written communications with a variety of people from different backgrounds and different levels of understanding, and present a positive image of the VA to the community. Ability to interpret and apply regulatory and procedural requirements to process unusually difficult and complicated transactions as it relates to the HBPC program. Ability to perform, plan and organize work in situations where numerous demands are made. Ability to identify safety issues related to patient/staff safety and acts appropriately to report unsafe conditions.
TYPING PROFICIENCY: This position requires the skills of a qualified typist. In addition to meeting the experience and/or education requirements, you must be able to type at least 40 words per minute.
