Nurse Practitioner in the Care in the Community service coordinates care in the community for services needs.
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan reimbursement program. If selected for this position, you must meet specific individual eligibility requirements and submit your application within four months of appointment.
Duties
The Care in the Community service coordinates care in the community for services that are not preformed within the VA medical center to include inpatient, outpatient, and emergency services. Nurse Practitioners assigned to the Care in the Community Service Line are expected to provide clinical guidance to all stake holders as required.
Performance Standards:
1. Takes a leadership role in utilizing problem-solving methodologies to resolve system and/or process problems that impact on resident care or staff performance at the program or service level that is supported by documented evidence of:
a. Assuming responsibility for meeting an identified need;
b. Developing and implementing the plan to meet the identified need; and
c. Evaluating and providing evidence of improved resident care or staff performance relative to the identified need.
2. Demonstrates role modeling, leadership and advanced clinical expertise to improve the delivery of patient -centered care using the nursing process for patients requiring specialized/complex care.
3. Develops creative, innovative strategies to resolve problems and/or promote positive outcomes in collaboration with nursing/medical center leaders.
4. Initiates, completes, and follows up on referrals, including consults and related referrals and requests for additional services as directed.
5. Identifies the need for new or revised policies, procedures, standards, or guidelines.
6. Leads interdisciplinary treatment team meetings to plan the care of the Veteran, and outcomes for discharge.
7. Completes orders and follows up on results, including lab, radiology, consults, medications, diets, code status and other needs to treat the resident.
8. Performs and documents reassessments, recognizing changes in patient's needs and re-establishes priorities for care, seeking assistance as necessary.
9. Evaluates patient education programs for groups of patients. Develops creative and innovating programs for patient and family education. Establishes patient/family teaching and/or counseling groups for specific patient populations with interdisciplinary involvement.
10. Utilizes interventions which promote patient's functioning capabilities (e.g., safely administers medications, utilizes techniques for management of patient behavior, provision of appropriate activities to enhance psycho-social and physical functioning of patient.
11. Includes patient/family in establishing realistic, measurable goals, planning care, and patient education, seeking assistance as needed.
12. Implements and documents interventions/orders in a timely and accurate manner, requesting assistance when needed.
13. Evaluates and documents patient response/progress towards attainment of goals/expected outcomes with minimal guidance.
14. Revises plan of care per evaluation, seeking assistance as necessary.
15. Adheres to policies and procedures in response to medical/psychiatric emergencies and initiates action, seeking assistance as necessary.
16. Follows established safety and infection control standards, policies and procedures with minimal assistance.
17. Actively participates in hospital or VISN wide committees.
18. Ability to review patients both clinical and administratively to determine eligibility for Care in the Community services.
19. Complete OPPE/FPPE as required, participates in peer review.
20. Provides guidance to all staff assigned to Care in the Community Service.
21. Maintain a relationship with self by nurturing self-knowing and self-care.
22. Deliver compassionate, quality care by ensuring interactions with all clinical disciplines are focused on establishing and maintaining healthy relationships.
23. Assists in the training of staff and other stake holders in the use of Care in the Community.
24. Assists in the coordination of care with outside providers using knowledge of the resources and specialty care available in the VA.
25. Facilitates ongoing communication between Care in the Community staff and leadership to evaluate data and review workload demands.
Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. The APRN successfully performs activities appropriate to their grade as outlined in the Nurse Qualification Standards. These Standards identify the educational, experiential and competency requirements for the APRN at each grade. Male and female Veteran patients of all ages are included in this area of practice. He /she must meet the competencies for this area of practice.
Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Master's or Doctoral Degree from a program accredited by the Accreditation Commission for Education in Nursing or the Commission (ACEN-Formerly NCLAC) or the Collegiate Nursing Education (CCNE).
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Nurse Practitioners must be licensed or otherwise recognized as a nurse practitioner in a State and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected.
Preferred Experience and Certifications - 1-2 years as a Nurse Practitioner
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.
Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.
