The Pharmacogenomics (PGx) Clinical Pharmacist Practitioner (CPP) at VA Sierra Nevada Health Care System (VASNHCS) functions at the highest level of clinical practice and licensure. The CPP functions as an advanced practice provider working independently, but collaboratively, with prescribing privileges as outlined in their scope of practice and approved by the Medical Executive Committee.

The CPP delivers evidence-based comprehensive medication management (CMM) through direct patient care. The PGx CPP provider will incorporate a "personalized, proactive, patient-driven" approach to care that empowers and equips Veterans to take charge of their health and well-being and to live their life to the fullest in accordance with overall VA initiatives. This includes, but is not limited to: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments that take into account the Veteran's PGx results, including deprescribing where clinically indicated. Additionally, the CPP helps achieve positive patient-centric outcomes through direct and indirect interactions with patients, providers and interdisciplinary teams in assigned areas; implements risk mitigation strategies; performs physical assessments; and orders laboratory and other tests to help determine efficacy and safety of medication therapy. The CPP manages medication therapy by initiating, modifying, and discontinuing medication both independently and in collaboration with other practitioners when needed. The CPP will follow patients in direct care setting to achieve therapeutic goals and make referrals as needed to optimize patient outcomes and treatment success. The CPP refers to the appropriate level of care for patients when needed and is responsible for documenting care of assigned patients.
Duties of the position include, but are not limited to:
Prescribe, monitor, and adjust medications under a scope of practice.
Obtain pertinent medical history, accurate medication-related history, and PGx family history from patient and via chart review.
Obtain oral informed consent and documents this encounter in the electronic medical record, as required.
Follow-up via face-to-face, telehealth, eConsult, telephone, secure messaging, and other modalities to provide patient care.
Review and verifies/processes medication orders for appropriateness and to reduce potential risk of adverse drug events.
Facilitate the creation of pharmacist-led specialty PGx services using either consult, e-consult, or care coordination agreement (CCA) functionality.
Review relevant population management dashboards to identify patients who may benefit from engagement in care, medication oversight or optimization, and/or risk assessments; and meet performance goals as determined by national, VISN, or local facility.
Apply knowledge to interpret laboratory values in the evaluation of patient care, recognizes significant abnormalities, and makes dose adjustments or recommendations based on objective laboratory findings.
Participate in the appropriate, effective and safe use of pharmaceuticals by contributing to criteria development, data collection, quality assurance and drug use evaluation and other quality & safety improvement activities for core practice area(s).
Advocate for the rational and ethical use of PGx testing as part of routine care.
Educate healthcare professionals using academic detailing frameworks about PGx principles and appropriate indications and contra-indications for cost effective PGx testing.
Provide resources and education that empower patients to make informed healthcare decisions about undergoing PGx testing and understanding their test result.
Lead diffusion of the PGx pharmacist train-the-trainer program to offer training to pharmacy leaders, clinical pharmacists, clinical pharmacy practitioners, clinical pharmacy Practitioners, and pharmacy residents. Administratively manage the PGx pharmacist train-the-trainer program tracking activity and report data to PGx National program offices, as required.
Facilitate PGx test results are handled in an ethical manner and that patients are provided access to their genetic data when applicable.
Participate in the appropriate, effective, and safe use of pharmaceuticals by contributing to criteria development, data collection, quality assurance and drug use evaluation and other quality improvement activities for core practice area(s) medication use and PGx.
Provide formal and informal education, lectures, and in-services to pharmacy, medical, nursing and other staff as requested.
Serve as a subject matter expert for assigned committees, workgroups or interdisciplinary teams.
Provide education to patients, their families and caregivers about medications, patient responsibilities, and information on health promotion.
Other duties as assigned by management.

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education:
Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: Clinical Pharmacy Specialist, GS-13
Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level.
Knowledge, Skills, and Abilities. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following KSAs:
Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
Expert understanding of regulatory and quality standards for their program area.
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Preferred Experience: Previous oncology experience and/or successful completion of a PGY2 oncology pharmacy residency. Pharmacogenomics experience or training.
