This position is located within the Section of the Behavioral Health Service Line at the Fayetteville VA Medical Center in Fayetteville, NC. The Caregiver Support Program's primary goal is to provide clinical evidence based services and interventions to caregivers, veterans and staff; provide education on caregiver issues and relevant VA benefits; provide community outreach; conduct resource development and provide evaluation/consultation on issues related to caregiving for veterans.
Duties

Duties include, but are not limited to, the following:
Develops, coordinates, and implements a comprehensive outreach initiative designed to facilitate the delivery of services to general caregivers and their veterans;
Maintains responsibility for coordination of referrals for general caregivers and their veterans;
Identifies and builds relationships with community partners and stakeholders;
Establishes and maintains a comprehensive list of community resources;
Gathers and organizes and facilitates the submission of all data for performance measures;
Assists with maintaining Caregiver Application Tracker (CAT) database;
Gathers appropriate clinical information from the medical chart to assist in the completion of treatment;
Performs telephone and walk-in triage;
Prepares files and charts;
Receives/screens and manages all incoming and outgoing documents;
Schedules conferences and meetings;
Tracks expenditures and referrals;
Performs other duties as assigned
Work Schedule: Monday-Friday; 8:00am-4:30pm

Qualifications
ENGLISH LANGUAGE PROFICIENCY: In accordance with 38 U.S.C. 7402(d), No person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. ~AND~
SPECIALIZED EXPERIENCE: One (1) year of specialized experience that equipped you the particular knowledge, skills, and abilities to successfully perform the duties of this position, and that is typically in or related to the work of this position. To be creditable, this experience must have been equivalent to at least the GS-6 grade level in Federal Service. Specialized experience includes answering multi-line phones, interacting with internal and external customers; triaging requests; typing a variety of correspondence; managing calendars; and scheduling conference rooms. Applicants wishing to receive credit for such experience must clearly indicate the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment.

Physical Requirements: The physical demands of the position involve standard office requirements such as using the telephone, retrieving files, writing reports, data entry and maintenance of computer databases, etc. Work is typically performed sitting at a desk and using a computer. There is no demand for extreme strength. There is stress involved when dealing with irate customers. Considerable walking between various buildings ofthe medical center is required. This position requires infrequent travel with other staff.
The work environment is primarily an office setting that is normally clean, warm and well lighted.
