This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws The Home Based Primary Care Pharmacist is responsible for providing clinical pharmacy services and managing all aspects of the Home Based Primary Care Program at the Edith Nourse Rogers Memorial Veterans Hospital, including but not limited to policy development, quality resources and coordination of educating staff to optimize cost effectiveness and patient outcomes. This includes participation on various committees pertaining to Home Based Primary Care, reviewing charts and drug therapy for patients, performing clinical assessments and interpreting lab data, providing drug information services including those services requiring extensive interpretation, providing pharmacokinetic consults as clinically privileged, processing prescriptions with distributive responsibilities, attending home visits to educate the patient and family on pharmaceutical care, and providing in-service educational presentations to medical center staff. This Clinical Pharmacy Specialist (Home Based Primary Care) position is located in the Pharmacy Service at the VA Bedford Healthcare System location. This position is a full-time at 40 hours per week. Duties for this position may include, but not limited to: Responsible for developing, organizing, coordinating and managing all aspects of clinical pharmacy functions for Home Based Primary Care, attends and participates in Home Based Primary Care multidisciplinary rounds, team meetings, conferences, and other meetings as required for the purpose of making recommendations and providing consultation in all Pharmacy-related matters; Responsible for all Home Based Primary Care patients registered in the program, must work independently and effectively to assure appropriate, accurate drug reviews are completed; interpret prescriptions and/or physician orders for accuracy, eligibility, availability, and cost effectiveness and therapeutic appropriateness; reviews patient medication profiles, contacts physicians and makes authorized changes in orders or prescriptions as required; Functions independently in day-to-day clinical and administrative duties associated with the position, consulting with higher level supervisors only when required; Demonstrates recognized expertise and knowledge in the area of geriatrics and adult medicine. Interacts with physicians and other health care professionals as demonstrated by regular attendance at Home Based Primary Care multidisciplinary team meetings, medication review rounds, and other meetings for the purpose of providing consultation in all Pharmacy related matters; Works closely with physicians and nurses to monitor patient response to medications, including efficacy, safety, and outcome, with special attention to polypharmacy problems in the elderly; Submits summary data or documents interventions on pharmaceutical care, costs, patient compliance, reduction in morbidity/mortality, selection of formulary products, and reduction in numbers of medications, etc.; Performs initial and periodic assessment of patients' medication therapy, monitors as needed, and makes recommendations or changes (within scope of clinical privileges) as necessary. Timeliness of reviews will be determined by JCAHO, HBPC, and /or other regulatory requirements; Demonstrates advanced knowledge of pharmacokinetics, pharmacodynamics and pharmacotherapeutics as determined by the clinical pharmacy specialist role and responsibilities; Provides verbal and written medication information to patients or families upon request. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Handbook 5005, Part II, Chapter 3, Section A, paragraph 3.g. Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate can provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence, or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. ?7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration, or certification. Grade Determinations: The grade of this position is GS-13, which is above the full performance level, you must meet the GS-12 Requirements and one full year of experience demonstrating the Knowledge, Skills and Abilities of the GS-12 grade. GS-12 Requirements: 1 year of experience equivalent to the next lower grade level, or Completion of an ACPE-accredited Pharm.D. program and Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate the following KSAs: Knowledge of professional pharmacy practice. Ability to communicate orally and in writing to both patients and health care staff. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. GS-13 Requirements: In addition to the GS-12 requirements, candidates must have 1 year of experience equivalent to the next lower grade level. AND Demonstrated the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: 1. Clinical pharmacy direct patient care experience in Geriatrics, Ambulatory Care or Internal Medicine within the previous 18 months. 2. Completion of an ASHP accredited PGY-1 Pharmacy Residency or equivalent experience. 3. PGY-2 Residency in Geriatrics or Ambulatory Care or equivalent experience. 4. Board Certification in Geriatrics, Ambulatory Care and/or Pharmacotherapy through the Board of Pharmaceutical Specialties.
