The New Mexico VA Health Care System is seeking a Supervisory Social Work Program Coordinator for the Health Care For Homeless Veterans (HCHV) Programs.

The objective is to re-integrate homeless veterans with complex psychosocial issues into the community and to help them achieve the maximum level of health and social functioning as possible. Achieve this goal of optimizing health and social functioning by actively seeking to identify eligible Veterans through Outreach efforts, conducting comprehensive assessments, and then providing appropriate interventions using community-based and VA resources. Responsible for the management of the various aspects of the Program to ensure compliance with applicable standards, to include metrics designed to monitor the quality of services provided. Supervises a significant number of professional staff at grades ranging from GS-05 through the GS-13 level. These positions include senior social workers, social work program coordinators, social work supervisors, occupational therapists, nursing staff, peer support staff, administrative staff, employment specialists, and other staff assigned to HCHV. Responsible for the management and coordination of the Homeless programs at the New Mexico VA Health Care System. This includes developing all aspects needed that will enable homeless Veterans to access case management, comprehensive medical and mental health care. Provides leadership to VOP staff in the application of clinical practice, case management, organizational processes and/or systems improving outcomes at the agency/service level, utilizing the clinical knowledge and skills required to provide general and mental health care outreach services to homeless Veterans. Networking and outreach to community agencies that provide services to homeless Veterans is a core component of this position. Community relationships are also an integral part of the HCHV Coordinator's responsibilities. These include actively participating in four statewide coalitions and participating as an impartial review member for the HUD Continuum of Care applications for the Albuquerque District, as well as other State and Local efforts targeted at Homeless Veteran populations. Ensures continuity of patient care in the development and implementation of the plan of care across all Programs. Facilitates evaluation and treatment of Veterans in contract facilities and participates in case conferences with those Veterans; plans will reflect continuity of care including expected short and long-range outcomes. Makes recommendations for filling clinical and other program positions, prepares the Homeless Program staff to perform as a functioning interdisciplinary team, provides training and consultation on a regular basis, coordinates and conducts training within the Homeless Program, coordinates staff development and in-service training for the Homeless Program staff, and participates as a member of the Social Work leadership body. Responsible for planning, coordinating and evaluating all aspects of the Homeless Programs by maintaining a program-specific database, budget, and answering suspense's that ensure the provision of quality care and continuity of service; represents the Homeless Program both within the NMVAHCS and the community at large; collaborates with community agencies and other VA programs to develop and implement services for homeless Veterans; oversees and supervises Grant and Per Diem contract and implementation of ensuing programs pertaining to the Homeless program to ensure compliance with VA policy, procedures and standards of care. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: Citizenship. Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part). Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to Accreditation | CSWE to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to VA's Title 38 Hybrid Occupations - Office of Human Resources Management. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Experience/Education. One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs. Skill in coordinating and implementing one or more specialty program(s), across the facility. Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to collaborate with internal and external partners to further program goals and enhance patient centered care. Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action. Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget. Knowledge of organizational structure and impact on program operations.
