This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
Duties
The VAMHCS Chronic Pain Service is a specialty service which provides consultative services for Veterans having conditions of chronic pain . This Service also has an interventional pain clinic and two telehealth programs - the Telehealth Empower Veterans Program (EVP) and the TelePain Clinical Resource Hub (CRH), a VISN 5 resource which is undergoing realignment. Located at the Baltimore VA Medical Center, the Chronic Pain Service is nested under Neurology Service, which consists of Outpatient Neurology Clinics, the Epilepsy Center of Excellence-Northeast Region, the Multiple Sclerosis Center of Excellence-East, and the Sleep Disorders Program. The Chronic Pain Nurse Case Manager (NCM) provides direct and indirect outpatient care to patients followed by the Chronic Pain Service. S/he is accountable for adhering to established VA and nursing policies, standards of care, and standards of practice. The NCM demonstrates leadership in providing care and improving clinical outcomes for Chronic Pain patients broadly and will also provide direct support to the interventional pain clinic.

The Chronic Nurse Case Manager (NCM) is responsible for facilitating the fulfillment of the mission, vision, and values of the VAMHCS Neurology Service.
The incumbent demonstrates leadership in delivering and improving care through collaborative strategies with other members of the healthcare team.
The NCM successfully performs activities appropriate to the grade as outlined in the Nurse Qualification Standards. The incumbent is responsible for consistent standards of nursing practice and of nursing care within the Neurology Service.
The NCM is responsible for the provision of high-quality, customer focused, cost effective patient care as it relates to his/her role within the Neurology Service.
The incumbent possesses a strong core knowledge base in nursing and has expanded clinic knowledge in neurological conditions, interventional procedures, chronic pain management, and case management.
The NCM may provide direct patient care including assessing, planning, health promotion, and preventive care.
The NCM may assist in diagnostic as well as interventional procedures.
The incumbent is also involved in indirect practice activities including staff and patient education, research activities, performance improvement, and consultation as related to management of acute and chronic neurological conditions.
The incumbent collaborates with APRNs, Nurse Educators, and health care providers throughout the VAMHCS.
The NCM coordinates patients scheduling, ensures patient safety and prompt follow-up through the case management process.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.
Preferred Experience:
Experience with case management of patients having chronic pain related conditions
Experience with case management of patients receiving interventional pain procedures
Medical and Surgical nurse experience

Physical Requirements: The position requires the incumbent to have visual acuity, keen hearing, clear distinctive speech, and manual dexterity. The incumbent must be mature, flexible, and sensible, capable of working effectively in stressful situations and able to shift priorities based on patient needs. This position requires limited periods of standing, stooping, bending, pulling, pushing, walking, and be able to lift up to 35 pounds. Transferring patients and objects may be required. This position also requires sitting for long periods typing on computer keyboard, looking at computer monitor, and using the telephone. The incumbent may be exposed to infected patients and contaminated materials requiring the incumbent to don protective clothing and equipment in isolation situations or during invasive patient procedures. The incumbent may be exposed to patients who are combative or aggressive related to neurologic or psychiatric disorders. The incumbent must meet the required screening/testing and be medically cleared by Employee Occupational Health as a condition of employment.
