This position is located in the Quentin N. Burdick Memorial Health Care Facility, Behavioral Health Department, Belcourt, North Dakota. The purpose of the position is to provide case management and intake activities for the Behavioral Health Department, and to assist patients with gaining access to medical, social, educational, and other services.

Duties

Serves as an interdisciplinary health care team member collaborating with other health care professionals in assessing patient/family needs and developing a comprehensive plan of care, ultimately coordinating and facilitating resolution of patient needs. Provides screening, planning, problem resolution, resource management, and facilitates crisis placement in crisis situations, emergency health care, and planning the discharge needs of the patient/family. Provides on-going supportive and/or case management. Monitors progress in treatment and informs Behavioral health team of progress. Tracks adult and adolescent in-patient referrals. Maintains current knowledge of in-patient treatment facilities admission requirements. Prepares referrals, and coordinates placement services for the patient and/or families with community nursing and group homes, in-home service vendors, and other related services. Provides continuity of care with each placement.

You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. BASIC REQUIREMENT(S): Social Science Series 0101 (opm.gov) Degree: behavioral or social science; or related disciplines appropriate to the position. or Combination of education and experience that provided the applicant with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. or Four years of appropriate experience that demonstrated that the applicant has acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below. MINIMUM QUALIFICATIONS: GS-7: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Provides direct case management, conducts intake services, and gathers demographic and screening information (e.g., suicidal, depression, anxiety); assesses patient eligibility to determine service needs; coordinates and documents in-patient referral process and assists clients with completion of admission requirements; monitors progress in treatment. Or, successfully completed one (1) year of graduate-level education that is directly related to the work of the position or superior academic achievement. GS-9: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Completes case management assessment on all clients; develops individualized, integrated care plan; coordinates placement services; makes referrals to help patients obtain needed and related services and activities; provides monitoring, follow-up, and evaluation; coordinates discharge plans as determined by the patient's medical condition, level of independence, functional capacity, family/social support systems, and financial resources. Or, successfully completed two (2) year of graduate-level education that is directly related to the work of the position. Time In Grade (Merit Promotion Candidates Only) Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP). You must meet all qualification requirements within 30 days of the closing date of the announcement.
