This position is located at the VA Central Western Massachusetts. The CNH/CRC team provides oversight for Veterans placed in the contract nursing homes and residential homes in Central or Western MA.The Contract Nursing Home (CNH) Social Worker serves as the Program Coordinator and primary clinician for a collaborative clinical / CNH and residential program/Community Residential Care (CRC).
Duties
The CNH social worker manages the daily operation of this collaborative program. He /She interacts with Veterans, families, facility staff, Office of Community Care (OCC) and contracting staff as necessary. The Social Worker is capable of practicing at the advanced level, has specialized knowledge, and at least two years of experience in clinical assessment, interpersonal skills, and communication techniques for all audiences required. The social worker should be licensed or hold certification in a state at the advanced practice level.
Clinically and administratively screens all referred Veterans for admission to the CNH and CRC programs.
Provides clinical/administrative intervention and case management for Veterans in the program.
Establishes a collaborative treatment plan with the Veteran, the CNH/CRC team and RN.
Coordinates and functions as the liaison for these CNH/CRC beds among team members, CNH/CRC staff and families.
Coordinates/oversees treatment with the community residential treatment partner.
Conducts ongoing assessments on a 30-45 day visit cycle in person and as necessary by phone.
Provides disposition planning and all related referrals.
Provides hospital-wide education about the program in order to ensure opportunity for access to all Veterans who meet admission criteria.
Participates in ongoing program development activities.
Organizes and participates in the oversight of the care thru viewing DPH survey results, The Medicare Star system and hands on facility visits.
Functions as the COR for the CNH/CRC program working with the contracting officials as necessary and the OCC.
Participates in the CNH/CRC Oversight Committee.
Participates in required social work and geriatric administrative meetings and trainings.
Participates on required committees.
Organizes the yearly Soldier's Home inspection with the VISN GEC Consultant, CNH RN and Soldier's Home staff.
Performs other related duties as assigned.

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in [both] spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
Education:
Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited.
A doctoral degree in social work may not be substituted for the master's degree in social work.

Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.

Grade Determinations:GS-12 Social Worker
Experience and Education:
Experience Requirement: Applicants applying at the GS-12 grade level must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.In addition to the experience above, the candidate must demonstrate all of the following KSAs:
Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
Ability to supervise multidisciplinary staff assigned to the program.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
Ability to provide training, orientation, and guidance within clinical practice.

Physical Requirements: Position requires movement throughout the clinic and/or Medical Center. Position requires travel to the community, patient homes and other sites. Demonstrates awareness of and follows all home visit safety guidelines.
