This position is responsible for the delivery of Whole Health (WH) Coaching and skill development of the local Medical Center's WH Coach program, including care through coaching, promotion and education. It supports Coaches and Veterans in the WH journey, specifically through the pathway, wellbeing, and WH clinical care elements of the WH System, by assisting in the collection of WH data from Veterans and creation of reports analyzing effectiveness of health coaching.

Duties

Health Coach Duties: The position works with individuals and groups throughout the facility in a client-centered process to facilitate and empower the client to develop and achieve self-determined goals related to health and wellness. Coaches support clients in mobilizing internal strengths and external resources and develops self-management strategies for making sustainable, healthy lifestyle behavior changes and more effectively managing chronic disease. While health and wellness coaches do not diagnose conditions, prescribe treatments, or provide psychotherapeutic interventions, they provide expert guidance and may offer resources from nationally recognized authorities. As partners and facilitators, health and wellness coaches support their clients in achieving health goals and behavioral change based on their clients' own goals and consistent with treatment plans as prescribed by individual clients' professional health care providers. Coaches assist clients to use their insight, personal strengths and resources, goal setting, action steps and accountability toward healthy lifestyle change. Health Coaches both enhance wellness and assist with management of chronic disease. Health Coaches may work in primary care as a member of a PACT to assist Veterans with weight management, smoking cessation, addressing problematic areas in the eight components of proactive health and wellbeing, and adherence to chronic disease such as diabetes, congestive heart failure and chronic pain. Health Coaches may work in specialty care-including mental health, nephrology, oncology, cardiology, infectious disease and others-to assist Veterans with identifying goals, developing behavior plans, and taking action steps to increase adherence to treatment. Health Coaches may work on inpatient units to assist with identifying goals, developing behavior plans for discharge, and assisting with post-discharge follow-up. In all settings, Health Coaches conduct interventions that lead to improved functioning, decreased probability of deterioration or admission, and enhanced quality of life. Promotion Potential: The selectee may be promoted to the full performance level GS-9 without further competition when all regulatory, qualification, and performance requirements are met. Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 08/24/2023. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-07 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. You may qualify based on your experience and/or education as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to Knowledge of health coaching structure, process, skills and techniques. Knowledge of ethical code and rules of conduct for health coaches. Knowledge of current research literature related to health and wellness, including behaviors that promote wellness and those that are detrimental. Knowledge of pertinent laws, regulations, policies and precedents to determine their impact on the customer service program and related support resources in the area studied in order to assist the Veteran in getting additional support or care when that is required, as well as navigate the personalized health inventory, personalized health planning, components of health and well-being, and existing VHA and community resources that support Veterans' personalized health goals, as well as other health and well-being programs available at a facility or in the community to support their personal health plans. In the developmental capacity the position must be familiar with and carefully follow procedures and insure standardized conditions. Interpersonal skills to present recommendations that improve patient services . OR, Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have . Successfully completed two (2) years of progressively higher-level graduate education leading to a master's degree or master's or equivalent graduate degree in a related field of the position to be filled. Education at the graduate level must have been obtained in an accredited college or university and must demonstrate the knowledge, skills, and abilities necessary to do the work of this position. Education cannot be credited without documentation OR, Combination: Applicants may also combine education and experience to qualify at this level. Combinations of successfully completed graduate level education (beyond the first year) and specialized experience as described above, to meet the total experience requirements. The education portion must include graduate courses that demonstrate the knowledge, skills, and abilities necessary to do to the work of this position. Physical Requirements: While some of the work is sedentary including researching, writing, program planning and data analysis, the position will have periods of extensive walking and movement throughout the healthcare system, prolonged standing, and transporting light to medium weight objects. The Employee Whole Health coordinator will also be required to travel between sites (CBOCs and outlying clinics) encompassed in the healthcare system. The work is conducted in a medical center environment and involves risk of exposure to a variety of contagious diseases. The work area is adequately lighted, heated, air conditioned, and ventilated.
