The Social Work Coordinator is responsible for planning, developing, implementing and evaluating one or more of the services, units or special programs at any of the sites of the NYHHS. Incumbent has coordination and administrative responsibility for the quantity, quality and effectiveness of the services provided by staff assigned to his/her service or unit.

Duties include but are not limited to:
1. The incumbent plans, coordinates, implements, monitors and evaluates the delivery of services within the scope of practice and administrative responsibility of the assigned program.
2. Monitors the efficiency, effectiveness, timeliness and quality of services offered within their scope of practice and area of responsibility pertinent to assignment.
3. Participates in compliance activities.
4. Participates in staff development program involving work with groups of staff around practice and learning needs.
5. Incumbent must be completely conversant with all levels of social work treatment and skill; and with group theory and practice, including new developments in the field.
6. Develops program guidelines and initiates new programs. Provides data and progress reports about the assigned area. Including quality management performance improvement monitors and any statistical or data analysis reports requested by management pertinent to assignment.
7. Acts to correct identified unmet standards. Is responsible for implementing day-to-day operational component of the psychosocial program within assigned area.
Coordination/Administrative Responsibilities:
1. Participates in the development of department policies and procedures to further social work goals and objectives; and in setting up the structure to ensure their implementation.
2. Incumbent plans and carries out regular workload reviews with all staff under their guidance to ensure appropriateness and effectiveness of social work treatment techniques, that appropriate priorities are being established and carried out, that cases are closed promptly when service is terminated and that recording standards are being adhered to.
3. In addition to their aforementioned responsibilities incumbent may be asked to provide coverage in other areas of the Social Work and Community programs.
4. Incumbent is responsible for providing help to subordinate staff in their management of their assigned units, identifying needs in practice through review of charts, 1:1 supervision, and staff meetings. Guidance involves giving advice, instructions and help in developing the needed skills in individual, family, group work and system intervention techniques.
5. If assigned social worker trainees, the incumbent collaborates with the Coordinator of the Student Program: in planning with the school of Social Work for field work content; in evaluating the trainee's progress; and in any way necessary to further the trainee's learning and growth.
6. Incumbent must have a comprehensive knowledge of the guidance/mentoring process and method; including understanding of individual learning theory, learning in groups and systems theories
7. Orients new professional staff and students.
8. Formulates individual development plan consistent with performance appraisal form and Service guidelines with all staff assigned to them.
Clinical Responsibilities:
1. The Coordinator has direct patient care responsibilities that include biopsychosocial assessments, treatment planning and follow-up of veterans identified as needing intervention, including time-limited individual , group or family treatment, case management, psychoeducational and supportive treatment.
2. Active participation in the interdisciplinary team in regard to patient care decision-making referral.
3. Specialized consultation for psychosocial issues, such as housing, entitlements,benefits, legal resources, transportation and other community services.
4. Utilizing telehealth forum as an assessment tool to ensure Veteran clinical needs are met.
5. Clinical coordination of care in collaboration with programs Listed in attachment A (is the programs listed up-to-date) aswell as the community.
6. Maintenance of statistical data and documentation are required by both program and professional standards.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
Social Worker (Program Coordinator), GS-12
Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
(b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
(c) Ability to supervise multidisciplinary staff assigned to the program.
(d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
(e) Ability to provide training, orientation, and guidance within clinical practice.
