The Social Worker Community Nursing Home Program (CNH) Coordinator position is in the Social Work service at the Bedford VA Medical Center, located at the Bedford, Massachusetts location. This position is full time at 40 hours per week. The CNH Coordinator oversees the daily operation of the program and directs patient care services in the program area.

Duties

The incumbent coordinates the day-to-day CNH placements, monitors the quality of care patients receive, monitors for adverse events in the CNH facilities and reports to the Chief Social Work and the Geriatrics and Extended Care Service Line Manager significant problems and concerns???????. Duties include but not limited to: Provides both informal and formal education to VA staff as well as community nursing home staff on any changes in VA policy concerning their specific programs; Prepares for internal and external surveys and maintains standards in accordance with accrediting agencies; Develops a systematic plan for reviewing and appraising the effectiveness of the CNH program in accordance with VA, VISN, VACO and local facility policy and procedures; Conducts CNH team meetings and Quarterly CNH Oversight meetings; Participates in national CNH Coordinator calls/meetings monthly; Recruit's community nursing homes for the CNH program as needed; Functions as a professional representative of this program and social work to CNH Administrators, local hospitals, Ombudsman, VBA, in contacts with other agencies and the public, and at conferences; Ensures adequate program coverage and makes changes accordingly; Ensures that work is completed in a timely manner and reflects quality documentation in accordance with Service, Medical Center, VA and JCAHO policy and standards; Documents all social work activity in the patient's medical record as appropriate: Demonstrates the ability to evaluate practice, services, program effectiveness, cost effectiveness and veteran and family satisfaction. Incumbent strives to reach optimal provision of quality of care, continuity of services and the most advantageous use of resources to benefit veterans; Performs work related to the review, monitoring, analysis, planning and assessment of CNH program; Travels, as may be required, is part of providing social work care management services to CNH veterans; and Assumes other duties as assigned. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. BASIC REQUIREMENTS: The basic requirements for employment as a VHA social worker are prescribed by statute in 38 U.S.C. 7402(b)(9), as amended by section 205 of Public Law 106-419, enacted November 1, 2000. To qualify for appointment as a social worker in VHA, all applicants must meet the following: a. Citizenship. Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). b. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. c. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grandfathering Provision.The following is the standard grandfathering policy for all title 38 hybrid qualification standards. Please carefully review the qualification standard to determine the specific education and/or licensure/certification/registration requirements that apply to this occupation. e. Physical Requirements.See VA Directive and Handbook 5019, Employee Occupational Health Services. f. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f). Grade Determination: Social Worker (Program Coordinator), GS-12 Experience. Coordinators have experience that demonstrates possession of advanced practice skills and judgment. At least 2 years should be in an area of specialized social work practice. A doctoral degree in social work may be substituted for the require 1-year professional social work experience in a clinical setting. Licensure. Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirement may be found on the OHRM website. Coordinators/Senior social workers must be licensed or certified by states are of specialized social work practice. Coordinators/Senior social worker must be licensed or certified by a state at the advanced practice level which included and ASWB advanced generalist or clinical examination., unless they are grandfathered by the state in which they are licensed to practice at the advance practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice). Coordinators/Senior social worker may have certification or other post masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. (3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. Preferred Experience: Incumbent demonstrations advanced level of practice skills and abilities, with specialty focus in geriatrics. References: Meets the qualification standard for the GS 12 Social Work Program Coordinator as defined in VA Handbook 5005, Part II Appendix G39, and Social Work Qualification Standard GS-185 Veterans Health Administration.
