The Captain James A. Lovell Federal Health Care Center (FHCC) is seeking a Patient Safety Program Manager, Not to Exceed 2 years for Quality and Patient Safety. This position is assigned to the Command Suite Directorate at the FHCC in North Chicago, Illinois.

The Patient Safety Program Manager develops, implements, analyzes, recommends and evaluates various initiatives and activities. Responsibilities include oversight for all patient safety related activities including Root Cause Analysis (RCA) of any event or near miss; and conducting proactive risk assessments, including Healthcare Failure Mode and Effects Analysis (HFMEA). Additional activities are to include investigation, tracking and trending of facility patient event reports, product safety reports/recalls, and other patient safety/risk management related problems.
The Patient Safety Program is a vital component to the FHCC's overall Performance Improvement program. Clinical expertise and knowledge of patient safety concepts are essential to ensure compliance with standards established by FHCCVHA/DoD directives. The Patient Safety Assistant helps to support the QM/PI mission of the FHCC.
Patient Safety Program data provides a basis for identifying problem areas and opportunities for improvement, and for establishing organization-wide policy instructions and procedures. The PSM analyzes the data and prepares meaningful reports for information, action, or further study by FHCC leadership, committees, teams and other FHCC personnel as appropriate.
Major Duties and Responsibilities
Coordinates, plans, and implements delegated aspects of the Patient Safety Program. This includes serving as a functional expert and resource person for the FHCC for activities related to the development and monitoring of patient safety- related processes. Assesses actual/potential patient safety issues using a proactive risk control plan to avoid, prevent or limit risk liabilities and/or loss and to stimulate change to avoid medical errors and improve safety.
Reviews, investigates, and intervenes on patient event reports, RCA, HFMEA, oral reports, and/or other written documentation that identifies patient safety issues. Provides prompt acknowledgement of reports and timely feedback to relevant personnel and facilitates development of action plans to prevent recurrence.
Compiles, maintains, and analyzes data; identifies problem areas or opportunities for improvement through trend analysis and prioritizes areas warranting further evaluation.
Assists with routine status reports to the leadership. If care/service is determined to be outside of the acceptable standards of care, ensures that follow-up action is taken.
Submits information and reports required by directives to senior management, and other authorities as directed by the Patient Safety Supervisor and/or the Chief of QM.
Participates in network-wide projects, as assigned, and consults as a member of network Patient Safety Council.
Evaluates and prioritizes problem areas. Presents reports to Quality Council, as appropriate, on the status of Patient Safety Program. Performs additional investigations, to include RCA and Healthcare FMEA, as required on issues involving quality of care, risk and patient safety concerns.
Plans for personal growth as related to professional goals based on self- assessment, evaluation, and feedback. Assumes responsibility for acquiring knowledge and experience to meet goals. Actively participates in professional organizations. Provides guidance and information on patient safety related topics to providers and staff at the FHCC. Conducts training for professional and paraprofessional personnel within the department and the FHCC.
Participates in review preparations conducted by internal and external organizations including, but not limited to, Command Evaluation, VA and Navy Inspector General, Navy Medicine East, VHA, the National Center for Patient Safety, and The Joint Commission

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.
Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.

Physical Requirements: To perform in a fully successful manner, the individual must have the physical ability to perform job-related duties which include light lifting (under 15 pounds) and moderate carrying (15-44 pounds), prolonged sitting (up to 8 hours), use of fingers and both hands, near vision correctable at 13" to 16" to Jaeger 1 to 4 and hearing (aid permitted). In addition, the following administrative duties must be performed: word processing, computer keyboard typing, computer screen viewing, answering/ placing telephone calls, filing, and note taking. The employee maybe exposed to the following: working alone or working closely with
others. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. A health examination must be successfully completed prior to this assignment and periodically thereafter as required by the Occupational Health Clinic policy. There will be intermittent exposure to blood and body fluids.
