The inpatient clinical pharmacy technician for medication reconciliation is a certified pharmacy technician with the ability to support the inpatient medication reconciliation program at the West Palm Beach VA Healthcare System (WPBVAHCS).
Duties
The incumbent is responsible for performing medication reconciliation for patients under the direct oversight of the Clinical Pharmacist Practitioners (CPPs) in the following areas but not limited to: Acute Care, Intensive Care, Emergency Department, Acute inpatient psychiatry, and Community Living Center. The incumbent will directly report to the Clinical Pharmacy Supervisor. The primary function of the medication reconciliation pharmacy technician is to assure and document the complete reconciliation of all medications. Other duties may include laboratory value monitoring, medical chart / action profile poly-pharmacy reviews, medication utilization evaluation monitoring, pharmacoeconomic reviews, telephone logistical support, and supplemental patient education communication. The medication reconciliation clinical pharmacy technician successfully and consistently demonstrates the exercise of independent technical judgment (without the need for readily available supervision) as well as comprehensive and thorough working knowledge of the most complex or difficult practices and procedures.
Some duties are, but not limited to:
Medication Reconciliation
1. Identify all new admitted patients in order to complete initial medication reconciliation.
2. Interviews patients (and families/caregivers as appropriate), to develop an accurate list of medications, strengths, and dosages the patient has been taking.
3. Interview patients for any over the counter medications and/or herbal products.
4. Review Joint Legacy Viewer (JLV) to see if patient has been receiving medications from another VA.
5. Contact other pharmacies to complete the accurate list if necessary.
6. Document adverse reactions and allergies that the patient reports.
7. Review any patient compliance issues based on the interview and refill history.
8. Compare and document the compiled list with the active inpatient medication profile in the Computerized Patient Record System (CPRS).
9. Add the CPP as an additional cosigner to the medication reconciliation note in CPRS.
10. Notifies the overseeing CPP of any discrepancies, omissions, questions, or concerns regarding the patient.
11. Contacts and collaborates with other healthcare providers in the facility as deemed appropriate.
Clinical
1. Review all encounters daily and notify inpatient CPPs for completion.
2. Review all consults and notify inpatient CPPs for completion.
3. Review all unsigned notes and notify inpatient CPPs for completion.
4. Review all user alerts and notify all inpatient CPPs for completion.
5. Review performance improvement dashboards which include but not limited to COPD exacerbation, heart failure exacerbation, uncontrolled hypertension, uncontrolled diabetes and reporting to the Clinical Pharmacy Supervisor.
6. Monitor all non-formulary medication documentation to ensure we follow the National standard for timely turnaround.
7. Reviews laboratory monitoring reports for selected disease states, identifies patients with lab values beyond normal limits, and reports these patients to the assigned inpatient CPP for further evaluation if needed.
8. Provides education for patients concerning operational issues or need for therapy compliance.
9. Prioritizes operations and assigned clinical functions to provide the highest quality of patient care
10. Participates in quality improvement processes and programs as assigned by Clinical Pharmacy Supervisor.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education or Experience. A high school diploma or equivalent; or at least 3 months experience.
Certification. Certification by the National Pharmacy Technician Certification Board (PTCB) is required at grades GS-6 and above. This certification IS required for this position. You MUST submit documentation with your application package.
English Language Proficiency. Pharmacy technicians appointed to direct patient care positions must be proficient in spoken and written English in accordance with chapter 2, section D, paragraph 5a, this part.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Please note: This position is above full performance level, therefore grandfathering is not applicable.

Preferred Experience: Experience with medication reconciliation is preferred.

The full performance level of this vacancy is GS 7. The actual grade at which an applicant may be selected for this vacancy is GS 7.
Physical Requirements:
The work requires regular and recurring physical exertion, involving standing for prolonged periods of time, lifting/carrying of pharmaceutical supplies weighing at least 50 lbs., and bending, stooping, and crawling. Incumbent must be physically and mentally able to perform efficiently the duties of the position.
