The position reports to the Chief Of Quality, Safety & Value. Within a framework of priorities set by the safety assessment code and the resources of the facility, the employee and supervisor develop timelines for patient safety activities, primarily including Root Cause Analyses and proactive activities while maintaining the quality of effort. The timeline and effort for these are usually set by multiple factors and input by the RCA Team as well.
Duties
Duties include but are not limited to:
Utilizes client care information based on assessment data collected and interdisciplinary collaboration with client/significant others.
Evaluates effectiveness of planned interventions, and outcomes. Solicits constructive input from all disciplines.
Functions as a resource at unit/team/work group level for standards development to meet medical center performance measures.
Initiates change of clinical practice based on root cause analysis recommendations and performance measure outcomes.
Formally shares information gained from continuing education experiences within the unit and/or within the Medical center.
Evaluates and shares research findings pertinent to area of practice in an attempt to update, revise or validate practice.
Identifies potential problems involving resources and/or safety and takes action to avert or manage the situation in a manner that meets the needs of clients in a safe/therapeutic manner.
Proposes/implements/facilitates actions to resolve problems identified through the QM/Patient Safety activities.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience: 2-4 years experience in critical care or Med-Surg nursing, 2-4 years experience in a leadership role, certified in patient safety or quality management, NCPS program experience.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.
Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.

Physical Requirements:
The work is primarily sedentary, although some slight physical effort may be required. This position requires writing or otherwise working with fingers and/or hands, including one or both hands frequently. This position requires the expression of ideas by means of the spoken word, including the provision of education and instruction to patients and the sharing of information with co-workers. This position requires the perception of sounds at normal range, including the reception of oral communication shared at interdisciplinary team meetings and oral requests from patients. Occasional kneeling and crouching is required by bending the knee(s) to come to a rest or kneel in order to assume an appropriate-level position for retrieval of items. This position requires the ability to see near and far, requires depth perception and the ability to distinguish between basic colors and shades of colors. This position requires using the upper extremities to exert force in order to draw, drag, haul or tug objects in a sustained motion, as in pulling equipment from one area to the next. This position requires the application of pressure to an object with the fingers and palm, as in the holding of a pen to jot notes, messages, etc. Frequently, this position will require sitting and the maintenance of a seated position, as when seated to work
at the computer and while participating in meetings. This position requires moving from one place to another on foot, utilizing one or both legs, and/or utilizing both legs and arms to change one's position in space, either upward or downward as in when climbing the stairs between floors. This position requires bending the body downward to a position of being on ones hands and knees to assume appropriate-level position to retrieve items such as pens that have fallen under equipment. Other environmental factors include being both inside and outside occasionally; excessive, intermittent noise; exposure to cleaning solvents; electrical energy; slippery or uneven surfaces; working around machinery or moving parts; working around moving objects or vehicles; fatigue factors related to stress; working with hands in water; working closely with others; and protracted or irregular work hours. This position requires the ability for rapid mental and muscular coordination simultaneously. Work is typically performed in an adequately lighted and climate controlled office. May require occasional travel.
