The social worker is primarily responsible for the management, coordination and development of a guardianship program for the Eastern Colorado Healthcare System. The incumbent will be required to serve as the subject matter expert for scope of legal guardianship for the Care Management and Social Work service. The social worker must be a skilled communicator capable of developing effective partnerships with local, state and federal agencies as well as non-governmental organizations.
Duties
Duties include but are not limited to:
Supports efforts of inpatient case managers discharging Veterans requiring legal guardianship from inpatient stays to a variety of settings.
Assess copayment requirements, placement and referral eligibility.
Coordinates with Veteran, family, VA and community stakeholders.
Interfaces with community agencies, providers and admissions coordinators.
Counsels and educates Advanced Directive, guardianship and conservatorship.
Provides various mental health and case management tasks such as provision of VA and community resources and referrals.
Evaluates risk assessment.
Facilitates family meetings.
Participates in interdisciplinary care teams.
Assists Veterans, their support persons or alternative options with guardianship acquisition.
Works with courts, law enforcement and other applicable institutions to ensure Veterans access to timely and comprehensive services.
Provides outreach and education to assist the Veteran, legal defenders, judges, VA clinical staff and other community stakeholders.
Consults and coordinates with Privacy Officers, Release of Information and Office of General Counsel on guardianship issues related to Veteran's privacy the sharing of information.
Provides education on the process to assess decision making capacity.
Understands the state process on obtaining a legal guardian.
Manages data related to guardianship cases.
Performs duties independently, exercising initiative and independent judgment in day-to-day services.
Supervises and consults of an administrative and professional nature.
Participates in consultation with the CMSW Section Supervisor or designee, Chief of Care Management and Social, as well as other authoritative resources at the Medical Center such as Regional Counsel, Business Services, Fiscal and Human Resource Management Services.
Provides supervision of social workers for advanced practice licensure or the graduate trainee program, act as a mentor/provides orientation for less experienced social workers, participates or chairs committees or groups that impact social work practice and/or lead roll out of evidence based practices.
Performs other duties as assigned.
Work Schedule: Monday - Friday, 7:00am - 5:00pm

Qualifications
Basic Requirements:
Citizenship: Be a citizen of the United States. (Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.)
English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f).
Education: Have a master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensed: Persons hired or reassigned to social work positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.
Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade.
Experience/Education: At least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment and are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs:
Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.
Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.
Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.
Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.
Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.
Preferred Experience and Certifications
Experience/familiarity with competency and capacity issues.
Ability to navigate complex systems within the healthcare system, community systems at the local, state, and federal levels.

Physical Requirements: Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. The Social Worker must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation if necessary, without endangering the health and safety of the social worker or others. Position requires walking throughout clinics, medical center and community, standing for extended periods of time, and involves sitting at a computer and in counsel and consultation with Veterans, families, caregivers, interdisciplinary teams and community stakeholders. At other times, the employee will be intervening with VA and community stakeholders on Inpatient Units and in the community. The incumbent is required to have a valid driver's license, and must meet VA requirements to drive a government vehicle. Travel within Denver Metro area is required.
