The social worker in this position functions as the Medical Center's Housing and UrbanDevelopment/Veterans Affairs Supportive Housing (HUD/VASH) Social Worker for the Health Care for Homeless Veterans (HCHV) Program. The social worker is responsible for providing home-based assessment, monitoring, maintaining and case management of higher acuity Veterans in the HUD/VASH Program as part of an interdisciplinary team.

Duties

Duties include, but are not limited to: Works with an interdisciplinary team to ensure care is coordinated with the VA Medical Center, community health/mental health and/or substance abuse providers and/or any other applicable agencies to meet the needs of the veterans utilizing the HUD-VASH Program either directly or via assignment. Practices independently at the advanced level and has specialized knowledge, skills and abilities of practice related to a particular diagnosis or special Veteran population such as the Homeless Veteran with highly complex health, mental health and/or substance abuse problems requiring expert level of psychosocial interventions. Helps the patient and family to understand the contributing factors to the problem(s), will discuss with them the pros and cons of possible short-term and long-term solutions, encouraging them to make positive and lasting changes to reduce stressors. Completes thorough assessments to determine the underlying causes of the presenting problem, the interpersonal and environmental factors impacting the problem, and its effect on the patient's ability and desire to comply with the treatment recommendations by the multidisciplinary team. Works independently with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, financial, legal and psychosocial problems. Participates as a member of the interdisciplinary treatment team and actively participate through collaboration with Veterans and family as well as interdisciplinary treatment team members in the development and implementation of treatment goals and interventions. Demonstrates a high level of skill and expertise to establish and maintain effective therapeutic relationships with Veterans in the HUD/VASH Program and/or their families. Completes thorough psychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their families and utilize this assessment in facilitating the Veteran's maximum use of treatment for attainment of the highest level of independence that is possible and practicable. Maintains knowledge of Veteran's benefits and services, community resources, and process for making appropriate referrals to community and other governmental programs or agencies. Utilizes principles of human growth and development over the life span and will be able to assist Veterans in coping with the loss and grief experiences from disability and terminal illness. Possess and demonstrate ability to communicate effectively, both orally and in writing with people from varied backgrounds. Supports the mission, policies, and procedures of the Department of Veterans Affairs, the Veterans Health Administration (VHA), the appropriate Veterans Integrated Service Network (VISN), and the facility. Possess a working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic and traumatic illnesses, substance abuse disorders, common medications and their effects/side effects, and medical terminology. Facilitates action for community placements through collaboration with Veterans and their families as well as interdisciplinary treatment team members to ensure that appropriate community placements are completed in a timely manner. Serves as a liaison between Veterans and/or their families and VA and community resources in order to ensure thorough delivery of services. Provides consultation and education to Veterans and their families regarding community resources, VA benefits and specialty programs, and advance directives. Develops a working relationship with the designated HUD point of contact and coordinates the housing voucher application and award process for each veteran. Maintains accurate records of admission and discharge dates of veterans utilizing HUDVASH vouchers. Provides consultation to other treatment team and staff members regarding psychosocial needs of Veterans and/or their families and the impact of the identified psychosocial problems on the Veteran's health care planning and compliance with treatment. Collects and submits veteran information as outlined by North East Program Evaluation Center (NEPEC) formats and procedures. Engages in coordination with the VA and community agencies, organizations, and groups as to learn of current services, resources and/or benefits available for homeless veterans and Actively participates in the HUD/VASH conference calls and participates in other relevant conference calls and/or meetings as assigned. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education.Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure.Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/ English Language Proficiency.Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. ? 7403(f). Grade Determinations:GS-12 Program Coordinator Social Worker. Experience/Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Assignment: For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS 12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts. Preferred Experience: At least 2 years of experience working with the homeless population. References: VA Handbook 5005, Part II, Appendix G39 dated September 10, 2019. Physical Requirements: This position involves primarily walking, talking, driving, use of the telephone etc. Moderate carrying and lifting, 15-44 lbs. Reaching above shoulders; Use of fingers; Walking 1-4 hours; Standing 4 hours. Operation of a motor vehicle to transport patients and materials is required.
