The Program Coordinator is a full-time position in the Social Work Department at the Northport VAMC, Northport, NY. The Northport VAMC has a large social work service that is located in a Level 1c tertiary teaching hospital including five (5) CBOCs and provides a full range patient care services (primary care, medicine, surgery, psychiatry, physical medicine and rehab, neurology, oncology, dentistry, geriatrics and ex. care) with state-of-the-art technology, education and research.

Oversees program planning and development, writing program operation policies/procedures and program guidelines, and implementing care processes; makes program recommendations to the Social Work Section Chief for Geriatrics and Extended Care based upon MFH program changes and work analysis. The incumbent has broad responsibilities for completing mandated reports, preparing reports and statistics for facility, VISN, and national use. Ensures that all program evaluation data is collected and entered at the appropriate databases; has extensive knowledge of Medical Center policies and procedures, VHA Handbooks and Directives, and VBA regulations. Collaborates with VA Medical Center multi-disciplinary teams and MFH Caregivers to strive for developing more effective and efficient ways of delivering healthcare services to Veterans utilizing MFH model of care. Oversees recruitment and development of the MFH Caregivers and homes. Utilizes the national directive, application forms, and inspection and tools to screen and assess Caregiver applicants wishing to become MFH Caregivers, which includes approving and disapproving MFH Caregivers based on VHA requirements. Coordinates the pre-approval inspections of all prospective MFH as well as the annual inspections of all approved MFH, to include coordination with interdisciplinary inspection team members, conducting the on-site inspection, compiling the survey report, and following up on all corrective action plans. Provides training to Caregivers, students, residents, interns, and fellows from other disciplines regarding the advantages and disadvantages of a MFH. Topics include but are not limited to Caregiver and family issues and the impact of psychosocial issues and problems of the Veteran in a MFH. Provides training to MFH Caregivers on topics related to the program. Coordinates training by other subject matter experts for MFH Caregivers. Veteran's functioning and health, assess risk factors, and determine acuity and need for services. Interprets and explains VA treatment and benefit programs as well as community based services to Veterans, facilitating referrals based upon Veteran's need and eligibility. Completes the initial assessment within 30 days prior to a Veteran's placement in a MFH. The assessment highlights the Veteran's strengths, limitations, internal/external supports, and service needs to optimize the Veteran's functional status and safely maintain the Veteran in a MFH. The incumbent conducts a home visit as part of the assessment. The incumbent develops crisis management plans, as appropriate, to maintain a Veteran in the MFH Program. Communicates and collaborates with the HBPC interdisciplinary treatment team regarding any concerns with the patient and/or caregiver. Provides consultation services to MFH Caregivers, family members, and other VA staff regarding the psychosocial needs of Veterans and the impact of psychosocial problems on health care and adherence to a treatment plan. The incumbent provides administrative oversight of MFH Caregivers, ensuring adherence to all VHA rules and regulations governing the program and the adequate and appropriate care of Veteran residents. Delegates work to VA staff assigned to the MFH Program. Reports to the Social Work Section Chief, Geriatrics and Extended Care, but practices independently, interacting with all levels of Medical Center and CBOC staff, community partners, Veterans, Caregivers, and family members where he/she has no access to direct social work clinical supervision. Strives to meet the needs of customers while supporting the VA mission. Consistently communicates and treats customers (Veterans, family members, visitors, stakeholders, community providers, and all VA staff) in a courteous, tactful, professional, and respectful manner. Qualifications

Basic Requirements: The following are the basic requirements and qualifications for appointment as a Social Worker in the Veterans Health Administration (VHA). Citizenship. Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens). English Language Proficiency Requirement - Per VA Handbook 5005, Part II, Chapter 3, Section A, Paragraph 3j: No person will be appointed under authority of 38 U.S.C., chapter 73 or 74, to serve in a direct patient-care capacity in VHA who is not proficient in written and spoken English. Education Requirement - You must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Note: Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree. Transcripts Required. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. A current, valid sate issued Driver's License is required for this position. Grandfathering Provision: The following is the standard grandfathering policy for all title 38 hybrid qualification standards. Please carefully review the qualification standard to determine the specific education and/or licensure/certification/registration requirements that apply to this occupation. All persons employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure or certification that are part of the basic requirements of the occupation. For employees who do not meet all of the basic requirements in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply: Such employees in an occupation that requires a licensure or certification, may be reassigned, promoted up to and including the full performance journey level, or changed to lower grade within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions. Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard. Employees initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration that meet all of the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation. If an employee who was retained in an occupation listed in 38 U.S.C. ? 7401(3) under this provision leaves that occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation. NOTE: If social workers covered under the grandfathering provision of the 1991 Federal law regarding licensure or certification of VHA social workers leave the GS-0185 social work series, they lose the grandfathering protection. If they choose to return at a later date to the GS-0185 series, they must be licensed or certified to qualify for employment as a social worker. Social Worker (Program Coordinator), GS-12: In addition to meeting the basic requirements as stated above, you must have the following experience: Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as Social Worker (Program Coordinator) must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments.
