The position is located in the Outpatient Clinic Departments, at the Fort Peck Service Unit. One position will be filled at each of the following locations: Poplar, Montana & Wolf Point, Montana. The incumbent of this position will report to the Director of Nursing.
Duties
Assess the patient care needs for the Primary Care Medical Home (PCMH) patients to include triage of acute issues for determination of appropriate level of care.
Provides direct care during face to face visits and telephone visits.
Develops a plan of care for individual patients utilizing nursing diagnosis.
Consults with primary care providers and other healthcare professionals on patients to coordinate diagnostic and therapeutic regimens for PCMH patients.

Basic Requirements
Education
A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education (external link), Council on Accreditation of Nurse Anesthesia Educational Programs (external link), Accreditation Commission for Midwifery Education (external link), or an accrediting body recognized by the U.S. Department of Education (external link) at the time the degree was obtained.
Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools (external link) is required for individuals who graduated from foreign nursing schools.
Licensure
For all grade levels and positions, applicants must have passed the National Council Licensure Examination (external link). In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below-
MINIMUM QUALIFICATIONS:
Basic Requirements for the GS-10 (or equivalent) Grade Level
In addition to the above requirements, applicants must meet one of the following requirements:
At least one full year of professional nursing experience (equivalent to the GS-9 grade level or above) and possession of a diploma, associate degree, or bachelor's degree in Nursing from a professional nursing program.
Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Management of nursing care in outpatient primary care environments and emphasizing interdisciplinary collaborative processes to assess, advocate, plan, implement, coordinate, monitor and evaluate health care options and services which are consistent with patient-centered medical home principles. Experience in coordination of patient care and resources for a caseload of patients across the continuum of care and through care transitions; Provide patient and family education pertinent to a wide range of conditions and health factors; Skilled in supporting patient self-management.
