This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. The physical therapist provides primary care physical therapy in the Physical Therapy Section of the Rehab and Extended Care Service Line at VA Health Care System in Dakota Dunes, SD.
CLINICAL FUNCTIONS : The Orthopedic Clinical Specialist assigned to Primary Care has responsibility for program development and program continuity in the area of Primary Care serving as a musculoskeletal expert. The physical therapist spends the majority of his/her time working in the Primary Care clinic area performing direct access Orthopedic examinations and treatment as well as helps guide Veteran care for musculoskeletal complaints requiring communication and coordination to/with several other disciplines in addition to the PACT team.
This individual demonstrates treatment competency that qualifies him/her to perform patient treatment procedures on Veterans with acute and chronic musculoskeletal complaints prior to being seen by their Primary Care provider or Specialist, i.e. Orthopedist or Neurologist. The physical therapist will function within a scope of practice to request diagnostic tests and changes to treatment pending approval notification of referring practitioner in case of adverse patient response. The physical therapist is viewed as an expert of musculoskeletal conditions in this setting by members of each PACT team, whose skills are beyond entry-level competence, functioning beyond the full performance level, in a direct access environment.
Establish goals, discharge plans, and referrals to other specialties based on evaluation findings and planning with the treatment team, patient, family, and/or significant other.
Develops and carries out physical therapy treatments as indicated to meet the patients rehabilitation objectives of decreasing pain and rapidly improving function. Conducts treatment interventions one on one with patients in PACT.
Independently solves problems and helps determine diagnosis not covered by the manuals, assists with triage to radiology and specialty clinics, adapts and modifies treatment plans, activities and procedures, without supervisory guidance, to meet complex and challenging needs of the patients evaluated in PACT.
Evaluate new adaptive aids, prosthetic and/or orthotic equipment. Coordinate with Prosthetic Department on restocking items in PACT and recommendations and appropriate modifications to enhance patient functional capabilities as evaluated in PACT. Determines patient's need for varied orthoses and prostheses and/or major/minor equipment warranting a consultation to amputee or major medical clinics for examination.
EDUCATION: Provides and arranges in-service training lectures on various treatment techniques, discusses patient designed treatment plans, and reviews algorithms for PACT Orthopedic referrals. Evaluates, revises and develops training materials, aids, methods and plans, being free to explore basic theories and principles of physical therapy, with students and other Rehab and Extended Care staff to include therapies, nursing, PACT providers, social work, psychology, integrative health and other ancillary staff.
Provide clinical training and supervision to physical therapy and PACT students. Assists other staff members in identifying needs and designing and implementing programs for assigned services as needed.
PROFESSIONAL/ADMINISTRATIVE: This individual is responsible for the monitoring of prosthetic equipment located in the physical therapist's office in PACT. Responsible for contacting Prosthetics Department to coordinate refilling stock as needed to promote Direct Access to assistive device training and dispersion. Items to include, single point canes, front wheeled walkers, standard rollators and Tens Units. This individual participates actively in service and section performance improvement activities; assists in developing Quality Management monitors and implements the same effectively and follows through with outcome measures. Will advise staff regarding safety, health, infection control and welfare of both staff and patients in treatment areas. This individual must be able to use daily monitors (such as active/pending consults) in carrying out scheduling of patient evaluation/treatment. Submits return to clinic orders and coordinates with MSAs to schedule patients who require follow-up visits in PACT, monitors caseload and communicates with supervisor when not meeting scheduling guidelines.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education and experience. The individual must meet at least one of the requirements below:
(1) Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist.
(2) Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a physical therapist.
(3) Doctorate degree in physical therapy.
NOTE: Prior to 1995 the terminal degree for PT's was either a bachelors or master's degree. In 1995 CAPTE accredited all baccalaureate, master's, and Doctor of Physical Therapy terminal degree programs. Effective 2002, CAPTE changed its scope of accrediting activities of PT education programs to include only those programs that culminate in post-baccalaureate degrees(eliminating the baccalaureate degree). Effective 2015, professional physical therapy education programs have only offered the Doctor of Physical Therapy (DPT) degree to all new students.
NOTE: Individuals must be a graduate of a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited college or university. The CAPTE is the only accreditation agency recognized by the United States Department of Education (USDE) and the Council for Higher Education Accreditation (CHEA) to accredit entry-level physical therapy programs. Verification of accredited programs may be obtained from the American Physical Therapy Association (APTA) at www.apta.org.
Licensure. Individuals hold a full, current, and unrestricted license to practice physical therapy in a State, Territory or Commonwealth of the United States, or in the District of Columbia.
English Language Proficiency. Individuals appointed under authority of 38 U.S.C. chapters 73 or 74, to serve in a direct patient-care capacity in VHA must be proficient in written and spoken English. See Chapter 2, section D, paragraph 5a, this part.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
GS- 13 Physical Therapist (Clinical Specialist)
Experience. In addition to meeting the basic requirements, one year of progressively complex experience equivalent to the GS-12 grade level, which is defined as: Making autonomous clinical decisions in a Direct Access environment. This includes ability to independently evaluate and treat patients who seek physical therapy services; Performing examinations and evaluations of individuals who have or may develop impairments, activity limitations, and participation restrictions related to conditions of the musculoskeletal, neuromuscular, cardiovascular, pulmonary, and/or integumentary systems while considering the effects attributable to unique psychosocial and environmental factors; Interpreting findings from examination and evaluation, select appropriate test and measures, and integrate findings into the physical therapy plan of care for the full range of patient populations; Establishing a diagnosis within the scope of physical therapy and identify the appropriate rehabilitation intervention, including referral to another provider for further consultation as clinically indicated; Determining physical therapy prognosis by incorporating examination findings with the patient's preferences in order to set clinically appropriate treatment goals, optimize outcomes and maximize functional independence; Independently providing clinical oversight of Physical Therapy Assistants as well as students on clinical affiliations who are in Doctoral PT Programs or PTA Programs.
And one of the following:
1. Evidence of a minimum of 2,000 hours of clinical practice directly in the specialty area. This time must be between the past two and five years; or
2. Board Certification in the specialty area recognized by the ABPTS; or
3. Completion of a fellowship program; or
4. An additional advanced degree in a related field.
In addition, the candidate must demonstrate the all following KSAs.
Demonstrated Knowledge, Skills, and Abilities
1. Advanced knowledge and expert skill in a range of specialized interventions and treatment modalities used in a specialized treatment area of physical therapy.
2. Ability to serve as a consultant and subject matter expert for health care providers regarding the delivery of care within a specialized content area of physical therapy (see paragraph 2i).
3. Ability to teach and mentor medical center staff in a specialized content area of physical therapy.
4. Ability to guide the work of a multi-disciplinary team in a specialized content area of physical therapy.
5. Advanced knowledge and expert skill to perform and interpret specialized procedures and tests in evaluating the treatment outcomes in the assigned specialty content area (see Definitions, paragraph 2. i.).

Assignment. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety. Clinical Specialists spend at least 25% or greater of their time working with specialized population(s) within their department. Physical therapists at this level perform assignments in specialty areas such as geriatrics, neurology, cardiopulmonary, orthopedics, spinal cord injury, amputee, wound care, chronic pain, electromyography, women's health or pelvic health, polytrauma, brain injury, lymphedema, vestibular, assistive technology, seating and mobility. The individual is assigned responsibility for serving as the subject matter expert in the content specialty area and as a consultant to physical therapy and other medical center staff in evaluating and treating patients in the specialty area. Clinical Specialists have advanced knowledge and demonstrate expert clinical practice skills in providing assessment and treatment interventions across the continuum of care. Incumbent utilizes advanced knowledge and expertise to modify treatment plans and to identify therapeutic activity interventions based on the changing needs, goals and performance of patients. Clinical Specialists have advanced training in specialized technology related to their area of practice. Incumbent establishes and maintains contact with other health care providers involved in patient care via written, telephone, and personal communication on a regular basis. Incumbent exercises expert professional judgment to establish appropriate discharge planning recommendations to ensure safe discharge from inpatient or outpatient rehabilitation programs. If the medical center has a PT residency program, the clinical specialist may be on faculty for the program.
