Provides services to Veterans referred for Residential Rehabilitation Treatment Program (RRTP) which includes three tracks: Post Traumatic Stress Disorder (PTSD), Substance Abuse, and the Psychosocial track. Coordinates all admissions for MH RRTP, and duties associated with the admissions process. Maintains admissions tracking including check in calls with individuals pending admission to meet the standards assigned by national VHA directives. Position is aligned under Social Work Service.

Duties

Coordinates all admissions for MH RRTP, and duties associated with the admissions process. Completes quality management functions, to include review of performance measures, policy and procedures, chart audits, and review and preparation for internal and external review processes. Completion of all preadmission clinical assessments. Treatment planning which includes, but is not limited to, referrals for further assessment, therapy services and/or inpatient psychiatric treatment, creating an integrated summary of the Veteran's medical, mental health, social, employment, financial and familial history. Responsible for provision of duties within the MH RRTP program on as needed basis. Responsibilities include provision of weekly group therapy sessions. Utilizes treatment evaluations to alter treatment plans to ensure effective clinical treatment, individual and group therapy sessions, crisis intervention, advocacy, and linkage to improve the Veteran treatment outcome and reduce mental health symptoms. Provide subject matter consultation to colleagues and students on psychosocial treatment of patients treated in the specialty mental health residential area. Assess and identify Veterans who have multiple issues including but not limited to: psychological and physical traumas, Post Traumatic Stress Disorder, Traumatic Brain Injury, substance abuse, dual psychiatric illnesses', physical disabilities and health concerns, etc.) that requires advanced therapeutic skill interventions to address an ever-changing group curriculum and/or individual session that will meet the specialized needs of each Veteran. Develop a psychosocial assessment of the Veteran in collaboration with the interdisciplinary treatment team, the veteran and family members/significant others. Responsible for contributing to the development of the treatment plan and setting achievable treatment goals with the Veteran, family members/significant others and other Outpatient Mental Health clinical staff. Reviews the progress notes from the other providers to accurately determine the strengths and limitations of each Veteran. Educates the Veteran, their family members/significant other and the team of all the options available to them and will collaborate with the Veteran and family members/significant other on the preferred option. Enters all Veteran /family contacts in the electronic record using appropriate formats and templates. This information will be entered in a complete, confidential, and professional manner to insure information on the Veteran is shared with other VA staff. Develop the mental health curriculum for the MH RRTP and enhance the system change with evidence base treatment models throughout each year based on the empirical findings. Leads the Quality Review Committee and complete documentation reviews. Provide education/outreach to providers and Veterans about the MH RRTP as well as the referral process. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. For this grade level, applicants must be licensed at the advanced practice level. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f). Grade Determinations: Social Worker (Program Coordinator), GS-12 Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Experience at the GS-11 grade level includes; Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. The environment involves everyday risks and discomforts which require normal safety precautions typical of offices, patient rooms, and in general, the medical center. It also involves normal risks associated with a medical center, including exposure to contagious diseases and disturbed patients.
