The Licensed Practical/Vocational Nurse (LPN/LVN) assigned to the George E. Whalen Primary Care Clinics functions as a team
member under the supervision of the Clinic Nurse Manager. This position will consistently be in a ready state to be deployed
to another VA site in the event of an emergency or critical situation requiring staffing support within a 72-hour notification
period. When not deployed, the LPN/LVN will support Primary Care PACT clinics in the capacity of a float nurse.
Duties
Observes, records, and reports changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan.
Collaborates with disciplines across the spectrum.
Provides patient driven care and administrative support to a Veteran population within the PACT care model for Primary Care.
Follows and contributes to a plan of care for individual patients and will work both as a team member and independently, communicating in a clear and constructive manner to members of the team, and clients both verbally and in writing.
Participates in PACT Huddles, team meetings, PACT trainings, and reviewing of PACT Compass / Data.
Ready for deployment of clinical staff dedicated to the continuity of Veteran healthcare and support to communities in times of crisis.
Be able to deploy, within the team's scheduled deployment windows, to any event within 72 hours of notification with the ability to work seamlessly with receiving facility or agencies.
Ready for an identified minimum of 2 rotational windows of expected deployable status per calendar year, for up to a month at a time.
Notifies Supervisor of any constraints that may impact deployment to any event within 72 hours, including but not limited to any protected leave entitlements and/or reported injury or illness, by following Supervisor notification processes and VHA Human Resources practices.
Work under a high stress healthcare environment, possibly in austere conditions, or in an emergency setting, while maintaining awareness of the critical elements of an emergency deployment by processing information from various sources.
Understand that CDT may deploy into austere conditions which are defined as: various housing and living conditions to include non-climate controlled, limited privacy sleeping facilities (for example, cots, tents, "hot-bunking", gymnasiums, or similar), limited shower and laundry facilities, and limited feeding options (such as shelf-stable meals, meals-ready-to-eat, and field kitchens).
Work may be performed in an office setting, indoor and outdoor field site, sometimes in adverse weather.
Agree to be deployed within their VISN outside of scheduled national CDT deployment windows.
VISN deployments may occur at the discretion of the VISN director with the network director keeping in mind the CDT staff's needs for rest, recuperation, and training.
Will not be deployed more than 120 days per year.
Provides direct patient care when deployed to an emergency, within the CDT staff's scope of practice/privileges/licensure/functional statement, with the resources available.
Ensure qualifications, required certifications, licensure, and registration particular to the clinical position are maintained and immediately report any issues/changes impacting qualifications, required certifications, licensure, and registration.
Maintains awareness of one's own behavior and is conscious of actions taken to support and or prevent compromising the mission at hand.
Complete mandatory CDT training courses prior to being deployed.
Demonstrate readiness to apply professional skills to a range of emergency situations during regular training and simulations. Participate in CDT specific training activities (e.g., conferences, exercises, simulations, specific competencies, certifications)
Maintain clinical competencies particular to the CDT role by participation in emergency response exercises and code teams.
Report to Occupational Health for physical fitness assessments for pre-employment and CDT-required pre-deployment reviews.
Meet the physical demands of the position which may require strenuous physical activity to include carrying persons and equipment, walking long distances over difficult terrain, running, climbing, working long hours in primitive or arduous conditions, long periods of standing, and bending and stooping.
Maintain VHA physical fitness requirements, vaccinations, and safety precautions related to infection control, and occupational safety requirements and preventative measures, such as personal protective equipment (PPE) requirements.

Qualifications

Basic Requirements:
United States Citizenship:

Education.
Graduate of a school of practical or vocational nursing approved by the appropriate State agency &/or [accredited by] the National League for Nursing [Accrediting Commission (NLNAC)] at the time the program was completed by the applicant.
Or
Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment.
Licensure
Full, active, current & unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia. An LPN[/LVN] who has or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the VA H&book 5005.
English Language Proficiency
Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in [both] spoken & written English as required by 38 U.S.C. 7402(d), & 7407(d).
Grandfathering Provision
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation & meet the criteria).

GS-4
Experience &/or Education
Employees may be appointed at or advanced to GS-4 if they meet any of the criteria:(a) Six months of qualifying experience as an LPN or LVN;
or
(b) Graduation from an approved school & one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure & sterile techniques & procedures.
or
(c) Graduation from an approved school of at least 24 months duration.
GS-5
Experience
Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent.
AND
Knowledge, Skills & Abilities
(a) Demonstrated knowledge & ability to provide a full range of practical nursing care to patients with a variety of physical &/or behavioral problems.
(b) Demonstrated ability to serve as a responsible member of the nursing team & interact [in an appropriate manner] with patients, family members, professional & [other] supportive personnel [involved in the delivery of] patient care, incorporating acceptable, established customer service st&ards into practice.
(c) Knowledge & skill sufficient to prepare, administer, & appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, &/or intravenous medications as permitted by approved local facility policies & procedures. Observation & documentation will include patient's response to medication administered & the reporting of any noted change in patient's condition to RN or MD/DO.
(d) Knowledge & ability to recognize the need for & to institute emergency measures when indicated, promptly seek the assistance of the RN or MD/DO & assist in resuscitation procedures in cardiac &/or pulmonary arrest.
(e) Recognizes & appropriately responds to breakage/malfunction or loss of equipment, safety hazards, & supply deficiencies, promptly reporting to appropriate personnel for corrective action.
(f) Completed work is under the general supervision of an RN or MD/DO.
GS-6
Experience
Completion of at least [one (1) year of [additional] qualifying experience at the GS-5 level or equivalent

(a) Technically proficient in initiating, performing & completing assigned duties in providing care to variable patient populations
(b) Knowledge & ability to appropriately carry out assigned patient care based on the patients' conditions; to use judgment in selecting the appropriate order & sequence of procedures & treatments; & to accurately recognize, report & record relevant] patient information.
(c) Ability to observe, identify & respond to the patient's needs for care, including medication, equipment-assisted care & patient/family education. In organizing & delivering care, the LPN/LVN recognizes & considers emotional, cultural, spiritual, socio-economic, & age-related factors.
(d) Prepares & administers prescribed medications oral, topical, subcutaneous, intramuscular &/or intravenous & performs treatments according to established policies/procedures. Observes for physical &/or emotional changes in patient's condition from prescribed medications/treatments, promptly & accurately documenting noted changes, & reporting any deviations from normal to RN or MD/DO.
(e) Knowledge & ability to recognize urgent or emergent patient care situations, seek assistance of the RN &/or MD/DO, & initiate appropriate emergency interventions as directed.
(f) Knowledge & understanding of human behavior, patient motivations & reactions to situations, & ability to appropriately utilize this knowledge in working effectively with patients, family members, & other staff.
(g) Establishes constructive relationships with individual patients & their families to elicit feelings & attitudes, & to promote positive relationships, communication & socialization skills. Fosters an environment of respect for individual patient & family rights to privacy & dignity in all aspects of care delivery. Effectively incorporates knowledge & understanding of established customer service standards in all interactions with patients, family members, &/or other internal/external customers.
(h) Knowledge & skill in performing support duties for complex diagnostic tests &/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing & h&ling specialized instruments or other specialized equipment, & monitoring the patient's condition before, during, & following the procedure. Serves as a preceptor in orienting, educating, & training less experienced LPNs/LVNs or NAs/HTs related to support duties for these more complex, specialized tests/procedures.
(i) Actively seeks out educational opportunities to enhance nursing knowledge & skills, sharing new knowledge gained with other staff to improve & advance nursing practice.

The full performance level of this vacancy is GS-6. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-3 to GS-6.
Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, & manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling & pushing a weight of 35 pounds in accordance with the Safe Patient Handling Program Directive 2010-032. Transferring patients & objects may be required. May be exposed to infected patients & contaminated materials & may be required to don protective clothing in isolation situations or operative/invasive procedures. May occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. Must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient & clinic needs.
