The Long Beach VA Health Care System is currently recruiting (1) Clinical Pharmacist Practitioner (Outpatient PACT Clinics). The incumbent will rotate through clinical areas in PACT service line including CBOCs and subspecialty PACT in accordance with Pharmacy Service policies and procedures, Pharmacy and Therapeutics Committee, Scope of Practice, established protocols, VA regulations, and State and Federal laws governing the practice of pharmacy. Provides medication management of chronic diseases consistent with their CPS Scope of Practice including medication initiation, dose titration or adjustment, discontinuation, assessment and therapeutic monitoring, patient education and medication adherence enhancement. Provides medication management consistent with their CPS Scope of Practice which may include, but not limited to, pharmacokinetics monitoring and management, antimicrobial stewardship, dysphagia support, immunizations, nutrition support, tobacco cessation, cardiac/renal/liver/endocrine/neurologic disease management, transplantation, and post-bariatric surgical care; management of anemia, anticoagulation, COPD, diabetes, hypertension, hyperlipidemia, hypothyroidism, obesity, osteoporosis in the following practice setting(s) performing any of the above via tele-health technology or in-person. Provides non-direct patient care activities include monitoring patient outcomes, analysis of adverse drug events, formulary management, population health management, teaching and research, quality assurance, medication utilization review and staff development. Competent in all computer programing and technologies that support assigned duties. Assists in the operation in other areas of Pharmacy Service, including distribution and clinical work in other areas (i.e., medication order review and verification) as assigned. Dispenses medications in accordance with dosage form requirements as assigned. Provides continuing education ( drug information, in-service, etc.) to peers, patients, physicians, nurses, pharmacy staff, and other healthcare professionals when requested. The incumbent actively participates in the formal and informal education of pharmacy trainees when requested by the Pharmacy Service to serve as a preceptor for trainees. Minimally, this involves planning, supervising and implementing graduate, undergraduate, residency, and internship programs offered in cooperation with affiliated institutions. Responsible for the overall management of the assigned clinic/ clinical area, including but not limited to developing and maintaining the clinic grids, maximizing and sustaining clinic utilization, managing missed opportunities, coordinating clinic cancellations, managing discharge planning, reducing medication-related re-hospitalization, and restructuring as necessary to effectively maintain patientcare in response to staffing needs. Participates in other duties as assigned by the Chief, Pharmacy Service and/ or direct supervisor(s ). Work Schedule: Variable shifts. Telework: Available Virtual: This is not a virtual position. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. BASIC REQUIREMENTS: Citizenship: Citizen of the United States Education: Applicants must possess one of the following: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).Grade Determinations:GS-13 Clinical Pharmacy Specialist: In addition to time-in-grade requirements, applicants must have 1 year of experience equivalent to the next lower grade level. In addition, applicants must meet the following Knowledge, Skills, and Abilities (KSAs): Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters Preferred Experience: Basic Life Support (BLS) certifications. Clinical pharmacy practice experience in PACT, ambulatory care, or successful completion of PGY2 Ambulatory Care Residency. Physical Requirements: Light lifting, under 25 pounds; Light carrying, under 25 pounds; Pushing (4 hours); Use of fingers; Both hands required; Walking (4 hours); Standing (8hours); Climbing, legs only (2hours); Ability for rapid mental and muscular coordination simultaneously; Near vision correctable at 13" to 16" to Jaeger 1 to 4; Far vision correctable in one eye to 20/20 and to 20/40 in the other; Hearing (aid permitted).
