The Robert J. Dole VA Medical Center (VAMC) in Wichita, Kansas is currently recruiting for a SOCIAL WORKER, Patient Aligned Care Team (PACT).

Duties

The Patient Aligned Care Team (PACT) -based model of care features three (3) major principles: patient-centered care, coordination of care and access to care. The Social Worker assigned to this position will act as an integral part of these PACT in the clinical and treatment care process. The incumbent is responsible for Social Work needs for PACT in Primary Care, which may include providing specialty care clinics at to the Robert J. Dole VA Medical Center. The incumbent provides a full range of services to patients and their families, who present with problems that range from basis psychosocial needs to more complex biopsychosocial needs. This position requires the ability to conduct psychosocial assessments of a wide variety of individuals from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds. Position requires a basic knowledge of human development and behavior (physical and psychological) and the differential influences of the environment, society, and culture. The incumbent will work with patients and families who are experiencing emotional, psychological, medical and social problems utilizing individual, group, and family counseling skills. S/he will possess the ability to assess the psychosocial functioning and needs of patients and their family members and to formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills and assistance needed. This position requires a basic knowledge of psychosocial treatment modalities and under supervision, the ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals; requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques. The incumbent will be expected to possess the ability to establish and maintain effective working relationships with clients, staff, and representatives of community agencies and the ability to communicate effectively, both orally and in writing, with people from varied backgrounds. Grade Determinations: In addition to the basic requirements as stated above, the GS-09 Social Worker must meet the following requirements: Experience, Education, and Licensure. None beyond the basic requirements. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: a) Ability to work with Veterans and family members from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds utilizing counseling skills (b) Ability to assess the psychosocial functioning and needs of Veterans and their family members, and to formulate and implement a treatment plan, identifying the Veterans problems, strengths, weaknesses, coping skills, and assistance needed. (c) Ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques. (d) Ability to establish and maintain effective working relationships and communicate with clients, staff, and representatives of community agencies. (e) Fundamental knowledge of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology. In addition to the basic requirements as stated above, the GS-11 Social Worker must meet the following requirements: Experience and Licensure. Completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non- VA experience) and licensure or certification in a state at the independent practice level. OR A doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting. Demonstrated Knowledge, Skills, and Abilities. Candidates must demonstrate all of the following KSAs: (a) Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. (b) Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. (c) Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. (d) Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. (e) Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. References: VA Handbook 5005, Part II Appendix G39 The full performance level of this vacancy is GS-11. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-09 to GS-11. Physical Requirements: The work is primarily sedentary but requires distance driving. It may require some occasional lifting (10-35lbs). Requires reaching above shoulder, twisting neck with ability to see from side to side with range of motion at least 45 degrees, and ability to coordinate all four extremities. Position requires frequent keyboarding (4-7 hours per day), sitting (4-7 hours per day), walking up to 6 hours per day, or standing up to 8 hours a day. Infrequent climbing/ descending of stairs and some bending. Ability for rapid mental and muscular coordination simultaneously.
