The Whole Health Program is an across the board change in the delivery of healthcare focusing on the patient in a proactive attitude vs reactive. The primary purpose of the position is to track and provide fact-based data to support the changes in the facility, to review the status of change, and to identify areas that can use focus for change and areas to receive praise for successes. The position is key to creating the business plan environment that will perpetuate the cultural change.

Duties

Duties include, but are not limited to: Briefs clinical Teams using VA provided data cubes and web page reports from the Corporate Data Warehouse (CDW). Reviews and applies data provided by Veterans' Experience data. Works with clinic support staff, clinic managers, clinical service chiefs, DSS staff, VISN staff and/or Office of Patient Centered Care & Cultural Transformation. Tracks the capture patient baselines and develops performance management tracking tools to further the proactive treatment of patient through Whole Health concepts and practices. Extracts, analyzes and provides data to assess treatment and services that impacts clinical endpoints, health status outcomes; estimated usage; characteristics of treating designated populations; evaluating patterns of substitution against other products such as resource impact. Serves as expert in the analysis of data across the service to include clinic productivity, national quality measures such as SAIL, and complex access correlations. Supports senior leadership through development and programming of reports and tools to manage access, quality and productivity. Produces reports of the delivery of Whole health across service lines and within clinics, and provider level quality and productivity reports. Prepares local reports utilizing ADHOC FILEMAN computer programming routines including MCAO, VSSC and OPCC&CT reports and local routines. Monitors and maintains profiles, identifies transmission errors by review of Veterans Health Information Systems and Technology Architecture [VISTA] records. Through data tracking, provides management with effective feedback that helps the organization operate successfully in a new personalized, proactive, patient-driven health care environment giving attention to both the experience of health care (e.g., healing relationships, communication, and environments) and the practice of health care (e.g., personalized health approach and components of health and wellbeing). Provides relevant clinical and/or administrative data to identify, quantify and resolve organizational problems. Use databases and registries formulates tools to support Whole Health patient care and ensure quality management. Provides for the maintenance and updating of report parameters and site-specific files by checking VISTA system clinic and provider setups, reporting changes in key performance indicators (KPI). Advocates the cultural change for delivery of care as called for by the Whole Health concept. Effectively and efficiently manages multiple competing priorities to ensure support is provided to all areas of whole health delivery system. Serves as the local expert on access data and correlates data for to support senior leadership decisions. Evaluates proposed changes or known deficiencies that have been identified and make recommendations to support new or revise policies and directives that will promote positive whole health outcomes Works closely with DSS/MCAO staff to ensure clinics are correctly aligned and coded for Whole Health tracking and costing. Maintains training and understanding of Whole Health implementation guidance. Applies statistical analyses and interprets the significance from MCAO, VSSC and OPCC&CT reports, databases and dashboards; including validity of measures used to generate outcomes related to patient participation in Whole Health. Develops and presents recommendations for improving clinical data-management methods, follow-up procedures, and timely compliance with regulatory guidelines. Collaborates with technical, clinical and administrative staff to develop system requirements while understanding system capabilities and limitations. Coordinates with Whole Health Field Implementation Teams and Whole Health director to develop strategies and plans for deployment of Whole Health. Develops and/or oversees databases related to patient management systems, and actively participates in or leads the development of Whole Health clinical decision-support systems. Provides advice and guidance to assist in revisions of operational procedures and clinical processes affecting Whole Health. Works closely with employees from all levels and all disciplines across multiple organizations, including leadership at the facility, VISN, or national levels, 01& T, commercial vendors, experts in the field, and others. Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 08/18/2023. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-9 position you must have served 52 weeks at the GS-7. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. You may qualify based on your experience and/or education as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-07 in the normal line of progression for the occupation in the organization. Reviews and applies data provided by Veterans' Experience data. OR, Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have successfully completed a masters or equivalent graduate degree or 2 full years of progressively higher-level graduate education leading to such a degree or LL.B. of J.D., if related that demonstrates the knowledge, skills, and abilities necessary to do the work of this position. OR, Combination: Applicants may also combine education and experience to qualify at this level. You must have an combination of specialized experience and education beyond the first year or 18 semester hours graduate level education in a subject area related to the position to be filled. Physical Requirements: Trips to satellite outpatient clinics or Department of Defense or other health care agencies are sometimes necessary and will involve transportation via station vehicle or privately-owned vehicle. The work performed is of a precise nature requiring long periods of concentration that is often interrupted. Employee is often faced with changing priorities and with meeting deadlines. The work involves carrying light items such as books, instruments, laptop computers and other similar materials.
