The Registered Nurse will assess, plan, implement and evaluate care based on age-specific components and assuming responsibility for the coordination of care focused on the transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences.

You will review each patient's individual plan in collaboration with the interdisciplinary care team to attain specific clinical outcomes in the outpatient specialty clinic settings. Duties of the position include but are limited to; Responsibility for Outpatient clinic coordination and patient flow management, including triage of patients, patient education, monitoring of patient progress toward predetermined goals and timely follow-up. Using sound judgement in assessing, planning, implementing, documenting, and evaluating age specific patient care using the nursing process. Utilizing nursing process to assess, coordinate, and facilitate care to patients throughout the continuum. Participating in the development and implementation of recommended nursing policies and procedures as related to designated clinical area. Collaborating with scheduling and clinic staff members to ensure patients are given timely appointments. Following up with patients, ensuring adherence to treatments plans, intervening in accordance with nursing policies and procedures. Answering patient communications and responds to messaging, voicemails and emails, addresses patient questions and concerns, focusing on customer service/patient satisfaction using active listening and professionalism. Addresses and resolves issues directly, whenever possible. Providing health care education to patients and family members seeking advice, with a focus on self-management, pre-procedure teaching, and wellness, utilizing the nursing process and triage per protocol and policies -Supporting direct and telehealth clinical programs within the organization and specialty area. Using a wide range of software and computers for video appointments, patient education, data review, and sleep/CPAP supply refill ordering. Actively developing and managing collaborative partnerships with community organizations and facilities, physicians, other clinicians, and interdepartmental relationships. Requests, reviews and documents outside records as needed. -Notifying patients of sleep study results and treatment options s/p provider's review, documentation, recommendations, and treatment plans. Documents accordingly. Sending appropriate correspondence to patients that includes results reviewed by treating provider, education material and treatment options. Leading and participating in process improvement initiatives. Work Schedule: Monday - Friday; 7:45am to 4:15pm or 8:00am to 4:30pm Work Schedule Statement: May be required to work other shifts, be on call, work weekends and/or holidays as patient care and needs of the service require. Should operationally needs dictate, candidates must be willing to work within other units (including community-based outpatient clinics, for example) to assist and support continuation of the provision of care/services. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Education: Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Licensure: Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: While not a requirement for consideration or selection, applicants with the following preferred education/experience will be considered highly qualified, BSN prepared Registered Nurse Minimum 1 year of Sleep Medicine experience. Minimum 1 year of experience in an outpatient specialty setting. Minimum 1 year of experience in care coordination and/or case management. Such experience must be clearly identifiable in the supporting application documents. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Lifting up to 35 pounds for 8-12 hours, intermittently; moderate carrying (15-35 pounds) for 8 to 12 hours, intermittently; lifting over 35 pounds with assistive devices in accordance with the Safe Patient Handling Policy except in emergent situations requiring prompt patient care; pulling for 8 to 12 hours, intermittently; pushing for 8-12 hours, intermittently reaching above the shoulders for 8-12 hours, intermittently, both hands required; walking 8-12 hours, intermittently; standing 8-12 hours intermittently; kneeling 8 to 12 hours, intermittently, repeated bending for 8-12 hours, intermittently; climbing and use of both legs and arms for 8-12 hours, intermittently, both legs required; near vision at 13 to 16" Jaeger 1 to 4, far vision correctable in one eye to 20/20 and to 20/40 in the other eye; ability to distinguish basic colors; hearing aid permitted; twisting, simply grasp and fine manipulation.
