This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws The Clinical Pharmacist rotates through each operational area including Inpatient and Outpatient at Pharmacy Service of the VA Caribbean Healthcare System (VACHS). The Clinical Pharmacist primary functions will be: interpreting provider orders, reviewing drug use and drug therapy, evaluating all provider orders, processing and/or entering information into the computer, providing final verification to all dispensed medications, counseling patients on proper medication usage, administration, storage and management. The Clinical Pharmacist disseminates and provides appropriate drug information to patients and healthcare professionals according to Pharmacy Service policies and procedures. The Clinical Pharmacist is also responsible for the following duties but not limited to: Interprets written or electronic orders utilizing professional knowledge, VA Policies and VA procedures, for oral and/or parenteral medications including chemotherapeutic or cytotoxic agents, IV admixtures, investigational drugs and controlled substances. Verifies and inputs orders into computer correctly. Evaluates all orders for drug interactions, incompatibilities, misuse, inadequate/inappropriate response, therapeutic misadventure and duplicate therapy. Checks inpatient/outpatient orders to include patient information, correct medication dose, route, and frequency of administration. Orders are checked for patient allergies, drug interactions, incompatibilities, duplication, and priority. Identifies, interprets, and resolves prescribing and dispensing issues, including pharmacotherapeutics and formulary adherence issues. Documents interventions as necessary. Encourages, facilitates and maximizes facility cost-saving proposals, including appropriate utilization of the 90 day fill initiatives, tablet splitting, etc. Provides patient education on medications including, but not limited to, any changes made in the current medication regimen. Using suitable oral, written or audiovisual communication techniques, verifies, through direct feedback, that the patient's or caregiver understands the purpose, proper use, and expected outcomes of the prescribed drug therapy. Documents education given in CPRS. Serves as preceptor and role model for pharmacy students, pharmacy residents and pharmacy technician students in accordance with established protocols. Supports the Performance Improvement Program within Pharmacy Service as required. Participates in interdisciplinary care teams and committees to improve quality of patient care. Supports Pharmacy Service Management by ensuring administrative compliance with various federal and professional requirements. Employee provides technical assistance regarding investigational drug use to the professional staff. Work Schedule: Monday - Friday 1:00 pm-11:30 pm/ Required to work Weekends, Holidays and on Call. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Clinical Pharmacist must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d) Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations Preferred Experience or Education: In addition to the basic requirements, candidates must meet one of the following: One (1) year of experience equivalent to the next lower grade level; OR Completion of an ACPE-accredited Pharm.D. program. In addition, Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs): Knowledge of professional pharmacy practice Ability to communicate orally and in writing to both patients and health care staff. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
