The incumbent, a professional licensed independent clinical social worker, functions at an advanced
level as a case manager for the VAACC's Housing and Urban Development/VA Supported Housing
(HUD/VASH) program. The HUD/VASH program is a specialized treatment program area, with
collaborative effort with the local Public Housing Authority to provide homeless veterans with
Section 8 Housing Choice Vouchers and long-term community case management.
Duties
Serves as a case manager for the HUD-VASH Program.
Responsible for data collection, administrative record keeping and monitoring patient or clinical indicators, thresholds, performance measurements and their outcomes.
Maintains working relationships and agreements with other organizations and directly
coordinates with the designated PHA regarding housing voucher application and award process for each veteran.
Participates in the review of agreements between Section 8 landlords, PHAs and the VA to establish appropriate referral sites for placement. May act as alternate point of contact for this VA facilities HUD VASH program.

Deputy Coordinator-HUD VASH Supervisor referrals and screening procedures for potential
HUD/VASH veteran participants that meet criteria for the program.

Regularly conducts and participates in outreach activities including field interviews, assessments and referrals for homeless veterans contacted in the community, shelters
and on the streets, as well as those who are referred by VA residential programs and facilities.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j.
Licensure. Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
GS-12 Social Worker(Program Coordinator) Experience, Education and Licensure
(1) Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
(2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.[(3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
(b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
(c) Ability to supervise multidisciplinary staff assigned to the program.
(d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
(e) Ability to provide training, orientation, and guidance within clinical practice.
Preferred Experience: Experience with maternal health care related to the Veteran population.

The full performance level of this vacancy is GS-12.
