The Chalmers P. Wylie VA ACC is seeking for GS-7 Licensed Practical Nurse for Grove City Community Based Outpatient Clinic. The Licensed Practical Nurse (LPN), GS-7 in Primary Care (PC)/Community Based Outpatient clinic (CBOC) provides skilled and individualized practical nursing care to all levels of patients including most complex patients who are acutely and/or chronically ill in the outpatient care setting.

Duties

VA offers a comprehensive total rewards package: VA Nurse Total Rewards The Licensed Practical Nurse at the GS-7 level has the knowledge and skills necessary for a concentrated effort and self -direction in carrying out the most complex practical nursing assignments. PATIENT CARE: The LPN provides nursing care to patients with chronic, acute, and terminal health problems, with minimal supervision of a registered nurse/provider; in addition, is responsible for primary nursing care to an assigned group of selected patients and is responsible for the plan of action in carrying out patient care assignments: Independently triages and prioritizes patient needs as emergent, urgent, or non-urgent in the clinic setting and modifies patient care/ treatment as needed. Will consult with provider or RN if they feel there is a decision outside of their scope of practice. Collects data on the physical (vital signs, weight, blood work, etc.) and psychological needs of assigned patients. Recognizes the need for and initiates action in emergencies, i.e., cardiac/respiratory arrest, seizures, and/or assaultive behavior. Knows how to therapeutically care for the mental health patient and provides appropriate intervention in the suicidal/homicidal patient. Works in collaboration with mental health and specialty staff and gives comprehensive hand off reports on the patient's condition. Correctly performs nursing care procedures and performs support duties for complex and specialized practices. Assists the provider and/or nurse with therapeutic or diagnostic procedures, including skin biopsies, joint aspirations, phlebotomies, pap smears, pelvic examinations, minor eye surgeries, and minor GU procedures. Obtains and arranges necessary equipment. CUSTOMER SERVICE: Promotes an atmosphere of caring by practicing positive customer service techniques through effective interactions with patients, visitors, and other personnel. Effectively incorporates knowledge and understanding of established customer service standards in all interactions with patients, family members, and/or other customers: Demonstrates complex communication and socialization skills that foster respect for patient/significant others rights to privacy and dignity in care delivery. Serves as a mediator for patient, families and colleagues to improve the quality of communication and interactions in selected care settings. Follows up on abnormal lab values with providers, meeting performance measures and evaluating team effectiveness in meeting performance measures. Receptive to guidance and suggestions, follows instructions conscientiously and completes assigned responsibilities accurately and on time. MEDICATION ADMINISTRATION: Has the knowledge/skill sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular and/or intravenous medications as permitted by policy/procedure. Demonstrates competency in medication administration by all routes according to established policy and procedures: Promptly completes all documents pertinent data in appropriate Cerner EMHR template or reminder using correct note title and linking the documentation to the correct clinic appointment or consult. Reports potential or actual medication errors to supervising RN and provider in a timely manner; follows agency procedures for reporting errors and EPERs. Provides patient/family education as appropriate on medication use including desired actions and adverse /allergic reactions. Independently runs and develops flu clinics, when indicated. ENVIRONMENTAL MANAGEMENT: Promotes a safe and therapeutic environment. Identifies potentially dangerous or emergency situations, and takes appropriate actions. Maintains precise adherence to basic principles of asepsis, safety, and universal precautions to provide a safe environment for patients, families, and employees: Follows hospital safety polices. Supports a safe and hazard free Environment. Demonstrates appropriate behavior to situations in which potential risk/danger exists for patients, visitors, employees or property, takes proper precautions to prevent injury to self, patients, others, and to property. Recognizes and responds to malfunction or loss of equipment, safety hazards, and supply deficiencies, promptly reporting to appropriate personnel for corrective action and work order entry. Work Schedule: Monday - Friday, 8:00 am - 4:30pm; Schedule may vary as deemed necessary to meet the needs of the Veterans and the Medical Center. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the Accreditation Commission for Education in Nursing (ACEN), previously known as the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Verification can be obtained from the State approving agency or the Accreditation Commission for Education in Nursing (ACEN), previously known as the National League for Nursing Accrediting Commission (NLNAC), 61 Broadway, 33rd Floor, New York, NY 10006. Exceptions include: Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment. VHA employees who were converted to appointments under 38 U.S.C. 7401(3) in implementation of Public Law 98-160, enacted November 21, 1983, and for VHA employees converted to appointments under 38 U.S.C. 7405 in implementation of Public Law 99-576, enacted October 28, 1986; and for their future personnel actions under this authority. The waiver will not apply to persons appointed under 38 U.S.C. 7401(3) or 7405 after these conversions. Individuals granted a license by a jurisdiction that does not require graduation from an approved school. Licensure. Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia. English Language Proficiency. Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in both spoken and written English. Minimum qualifications: 2-3 years successful nursing practice Preferred experience: One year primary care PACT LPN experience, in good standing and above satisfactory rating. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Of at least one (1) year of additional qualifying experience at the GS-6 level or equivalent, fully meeting all the performance requirements for the GS-6 LPN/LVN. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders.
