This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

The ICU RN is a professional nurse who is manages the care of the most critically ill patients in our hospital. This nurse must excel in critical thinking, problem solving, organization, collaboration and teamwork, and communication. The patient population is a wide variety of medical and surgical patients with a large number of cardiac and heart failure patients. ICU nurses must maintain a very large skill set to care for all patients and must stay proficient in several low volume, high risk patients which may require high risk devices such as cardiac support devices, CRRT, hemodynamically unstable patients, neuro patients, and ventilators. ICU nurses function as a member of a multi-disciplinary care team including physicians, pharmacists, social workers, physical therapists, occupational therapists, and speech therapists. ICU nurses provide whole patient care, meeting the physical, emotional, and spiritual needs of their patients. They are a resource in the hospital and can care for any patient that is admitted. ICU nurses often cover after hours recoveries and procedures, assist in difficult IV placement and feeding tube placement on all units across the hospital. They respond to codes as ACLS providers and provide support and education to other units who are less proficient with unstable patients.
The ICU supports a culture of learning and growth through education, participation in unit or hospital committees, and provides opportunities to participate in QI projects. The ICU encourages it's staff to continuously learn through on-going education, professional organizations, and advanced certifications such as PCRN and CCRN.
Major Duties:
Completes admission assessment on assigned patients within 24 hours of admission
Conducts reassessments on patients every 4 hours and as needed to determine rapidly changing conditions
Transcribes provider's orders using CPRS, checks orders in CPRS at minimum of every 2 hours
Takes verbal and telephone orders per policy
Completes patient care plans according to policy
Documents vital signs and assessments according to SOP using CPRS and/or Essentris
Documents medications using BCMA (Bar Code Medication Administration)
Plans for discharge/transfer of patient including completing discharge/transfer notes in Essentris/CPRS
Functions independently when performing for complex patients in the ICU
Initiates referrals and/or consults as appropriate to achieve interdisciplinary care of patients
Identifies crucial assessment changes in patient status and appropriately communicates these changes to provider and care team
Responds appropriately to emergent situations following ACLS guidelines
Safely performs moderate sedation, and out of OR recoveries per policy
Provides individual, complex patient care consistent with critically ill medical and surgical patients consistent with identified diagnosis and standards and scope of practice, which may include but is not limited to:
Cardiac Monitoring
Hemodynamic monitoring
Administration and titration of high risk cardiac and/or vasoactive medications
Ventilators, CPAP, BIPAP, and HF nasal cannula
Advanced cardiac devices (IABP, Impella, BIVAD, and LVAD's)
Advanced renal replacement therapy (CRRT)
Advanced neurologic devices (Lumbar drain, EVD, bolts
Post op management for cardiac, vascular, neuro, urology, general surgery, ortho, and ENT (not an all-inclusive list)
Cardiogenic, hypovolemic, neurogenic, and/or septic shock
Demonstrates ability to manage daily routine operations related to patient care (delegating, supervising, trouble shooting, and evaluating)
Participates in clinical supervision of nursing students and new hires through role modeling and communication objectives and expected competencies of orientee's and students

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience: Bachelors prepared Registered Nurse with 1-2 years of ICU Experience
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.
Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.

Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The individual may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The individual may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The individual must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. The individual who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles.
