This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
Duties
The Dayton VA Medical Center is recruiting for a well-qualified Senior Social Worker. The Acute Care Medical Social Worker is responsible for the management, coordination and provision of social work services to Veterans and their families on the Acute Medical/Surgical, TCU and ICU.
The Senior Social Worker possesses advanced practice skills and judgment with at least two years in an area of specialized social work practice. They assigned to a major specialty treatment program or to a special patient population with highly complex physical and/or mental health conditions requiring advanced-level psychosocial interventions. They should possess an advanced practice certification or other post- masters training in an area of specialized practice.
Duties and skills include, but are not limited to:
having a high level of clinical practice, knowledge, and leadership skills to improve patient access, flow, and positively impact the facility utilization of services
working independently in accomplishing assignments and program goals
managing, coordinating and providing of social work services
coordination of clinical and case management services and care with the various interdisciplinary healthcare teams and healthcare providers across acute care and geriatric services
using knowledge of utilization review criteria, clinical documentation requirements, current standards of care and compliance guidelines
advanced skills and knowledge about patient treatment and disposition
guiding the interdisciplinary team in the appropriate use of clinical services to enhance the continuity of care and process improvement
assessing, planning, implementing and evaluating the care of hospitalized patients
identifying and resolving barriers which may hinder effective patient care
engaging in process improvement activities that are focused on efficient care delivery and promote positive clinical and quality outcomes
supporting the successful transition of care by ensuring the patient understands and has the tools to follow the prescribed discharge plan(s) as well as follow-up with appropriate care provider when issues or concerns are identified
reviewing options and services with patients and families, including costs, alternatives, risks, and benefits
maintaining current knowledge of multidisciplinary resources, programs, and services
referring patients for community resources as appropriate
promoting cost-effective use of resources
providing and promoting patient-driven care in a safe, efficient and cost-effective manner, using evidence-based principles
improving processes and outcomes of care and shares information with staff, patients, and families
designing and revising appropriate documentation strategies to meet regulatory standards
participating in interdisciplinary meetings to facilitate collaboration and the achievement of identified goals
acting as a consultant to other members of the interdisciplinary team in the planning, implementation, and evaluation of patient care
collaborating to develop and implement programs to improve the care of patients with complex physical and behavioral problems
monitoring and educating staff on performance measure completion
perform quality of care reviews and quality improvement activities that result in improved organizational performance and patient care
assuming responsibility for initiating continuous quality improvement activities related to outcomes, resource utilization, and fiscal outcomes
analyzing need for changes in policies, procedures, and practice; make recommendations to management for change related to patient access, satisfaction, outcomes, clinical guidelines, performance measures, and regulations/standards
collaborating with internal and external customers to facilitate improvement within available resources
coordinating and providing care based on identified needs and/or results of customer satisfaction surveys
revising plans of care based upon the evaluation of variances, age-specific needs, and patient responses to interventions to improve patient outcomes

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j.
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website (https://www.cswe.org/Accreditation.aspx) to verify if that social work degree meets the accreditation standards for a master of social work
Licensure: Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
Senior Social Worker, GS-12
(1) Experience/Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.
(2) Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.
(3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs:
(a) Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.
(b) Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.
(c) Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.
(d) Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.
(e) Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.

The full performance level of this vacancy is 11. The actual grade at which an applicant may be selected for this vacancy is GS-12.
