The incumbent will be assigned as a Medical Instrument Technician (Polysomnography) and will be under the Pulmonary Section under Medical service. Will serve as a senior Sleep Technician and/or lead and will assist in scheduling, preparing, collecting, monitoring and assessing complex biological electroencephalographic and pulmonary data for multiple specific difficult polysomnographic procedures independent of direct supervision in accordance with the established guidelines.
Duties
The major duties and responsibilities include, but are not limited to:
Performs a variety of polysomnogragraphic studies in the sleep laboratory; routine; routine polysomnogram, PAP titration studies including, CPAP, Bi-level, specialized positive airway pressure technologies (including but not limited to servo ventilation, volume assured pressure support and timed devices), Home sleep testing.
High level of understanding of high and low filters, sensitivities, gain, AC/DC filters and inputs, bio-calibrations, machine calibrations, and instrumentation and electrical safety.
Assist with patient setup, calibrate machines, and collect acquisition performance verification.
Manually scoring of sleep studies. Score sleep/wake stages and scoring clinical events by site accepted guidelines.
Serves as a liaison to providers for equipment, supplies and request for special services.
Recognize the need for medical intervention if patient safety is at risk. Implement appropriate interventions as indicated if patient safety is at risk including alerting hospital emergency response teams, application of supplemental oxygen, initiation of ALCS protocol, etc.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: There are no specific educational requirements for this occupation. Education may be substituted for experience only at the GS-4 and GS-5 levels.
Licensure or Certification: Licensure or Certification is not required for this occupation; however, it is strongly desirable at GS-6 or above as evidence of possession of the essential knowledge, skills, and abilities. Basic and Advanced Cardiac Life Support certifications are desirable. Training which does not result in official certification does not meet this definition.
English Language Proficiency: Medical Instrument Technicians must be proficient in spoken and written English in accordance with chapter 2, section D, paragraph 5a, this part.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Preferred Experience:
Three years of current polysomnography experience.
Knowledge of respiratory equipment including positive airway pressure (CPAP/BIPAP) therapy.
Registration by the Board of Registered Polysomnographic Technologists (BRPT) is highly desirable.
Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) certifications are also desirable for employees at this level.
Grade Determinations: GS-09
At least 1 year of experience comparable to the next lower grade level GS-8, which is directly related to the duties of the position to be filled, and must fully meet the KSAs at that level. This would be experience which demonstrates possession of the knowledge required to provide all aspects of polysomnography services. In addition, the candidate must demonstrate the following technical KSAs and the potential to acquire the assignment specific KSAs. (MUST REFLECT IN RESUME)
Demonstrated Knowledge, Skills, and Abilities:KSAs 1, 2, and 3 are appropriate for all positions at this level.
Knowledge of the more complex examination and treatment procedures and techniques in order to provide training and supervision in these procedures, to evaluate and initiate performance improvement projects, and to write policies, procedures, and protocols, that pertain to Polysomnography.
Knowledge of JCAHO and other regulatory requirements.
Ability to communicate effectively and professionally with employees at varying grade levels.
Ability to provide, or provide for, staff development and training.

Physical Requirements: The work requires regular and recurring physical exertion such as standing or walking for prolonged periods of time; frequent bending, reaching, stooping, and stretching to set up and take apart equipment and apparatus; lifting and repositioning patients; and pushing or pulling heavy objects such as oxygen tanks and/or equipment.
The work may require specific physical characteristics and abilities such as above-average dexterity to perform intricate respiratory therapy / sleep procedures.
