The position is responsible for the delivery of Whole Health Coaching and skill development of the Cincinnati VA Medical Center's Whole Health Coach program, including provision of care through coaching, promotion, education regarding Whole Health Coaching to Veterans and staff. The position is integral to supporting Whole Health Coaches and Veterans in navigating the Whole Health Journey, specifically through the Pathway, wellbeing and Whole Health clinical care elements of Whole Health care.

Duties

The Cincinnati VA Medical Center is a two-division campus located in Cincinnati, Ohio and Fort Thomas, Kentucky serving 15 counties in Southwest Ohio, Northern Kentucky, and Southeast Indiana with 6 Community Based Outpatient Clinics located in Bellevue, KY; Florence, KY; Lawrenceburg, IN; Hamilton, OH; Clermont County, OH, and Georgetown, OH. Cincinnati VAMC also supports two off main campus locations in the Cincinnati area via an Eye Center and Mental Health Outreach Community Division. Duties Include: Contact eligible participants for enrollment and explain the health coaching process. Provides health coaching for Veterans in order to reduce or eliminate high risk behaviors while increasing healthy ones. Conducts interventions for Veterans using one on one, couple or group modalities in outpatient and inpatient settings. Conducts Whole Health facilitated groups' participate in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health and other inpatient and outpatient groups Provides follow-up care through telephone clinics or secure messaging between face-to-face appointments. Coordinates care with Veterans' treatment providers, both primary and specialty care, to ensure identified goals are consistent with best practices for treatment and management of chronic illness. Reviews and develops treatment plans for/with Veterans. Serves on healthcare system, VISN, and/or national VA committees or task forces specifically relating to Whole Health or Health Coaching. Assists with data management analyses, special studies and reports for use by management for budget preparation, workload projections, and quality assurance and utilization reviews. Provides training and troubleshooting for other health coaches in health coaching administrative processes and associated supporting issues. Conducts outreach activities to inform Veterans and family members of available programs Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level. The Whole Health Coach will also be required to travel between sites (CBOCs and outlying clinics) encompassed in the healthcare system. The work is conducted in a medical center environment and involves risk of exposure to a variety of contagious diseases. The work area is adequately lighted, heated, air conditioned, and ventilated.

Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 09/30/2023. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-7 position you must have served 52 weeks at the GS-05 level For a GS-09 position you must have served 52 weeks at the GS-07 level The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Proficient in spoken and written English in accordance with VA Handbook 5005 chapter 2, section D, paragraph 5a GS-7 Qualifications: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Providing health coaching and/or providing information regarding lifestyle management including knowledge of health coaching structure, process, skills and techniques; experience coordinating with others as part of a health care team; knowledge of programs such as weight management, nutrition, exercise/fitness, smoking cessation, stress management, diabetes, pain management, etc.; experience developing treatment plans that focus on behavioral interventions; assist with data management analyses, special studies, and reports for use by management for budget preparation, workload projections, quality assurance and utilization reviews; knowledge of medical terminology; skill in the use of computer equipment with a variety of software applications including the ability to produce documents, reports, and educational materials; skill in the use of Microsoft office software and other databases to track, retrieve, and share information; and demonstration of skill in communication and interpersonal relations both orally and in writing providing clear and concise communication. OR, Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have successfully completed (1) year of graduate-level education or superior academic achievement. GS-9 Qualifications: Specialized Experience: At least one (1) full year of specialized experience equivalent to at least the next lower grade level (GS-7) in the Federal Service, that is directly related to the work of the position and has equipped you with the particular knowledge, skills and abilities to successfully perform the duties as a Social Science Program Coordinator. Qualifying specialized experience includes: reviewing supporting material to determine compliance with regulations and policy requirements; assists in planning and implementing program(s) within a medical center setting; using creativity to engage employees and community partners in a newly developed program; developing training materials for staff educational needs; and experience using a wide range of qualitative and quantitative methods for the assessment and improvement of program effectiveness. OR, Education: Successfully completed two (2) years of progressively higher-level graduate education leading to a master's degree or master's or equivalent graduate degree in a behavioral or social science; or related disciplines. Education at the graduate level must have been obtained in an accredited college or university and must demonstrate the knowledge, skills, and abilities necessary to do the work of this position. OR, Combination: Combinations of successfully completed graduate level education (beyond the first year) and specialized experience as described above, to meet the total experience requirements.
