The Physician - Care In The Community (NP-CITC) will be in daily contact with the Care in the Community staff with regards to community care related activities. The NP will have intermittent contact with their supervising physician/Deputy Chief of Staff about patient care activities and for additional clinical support/guidance.
Duties
The Physician - Care In The Community (NP-CITC) will:
1. Serve in the management of Community Care consults and request for additional services as directed.
2. Demonstrate sound decision-making skills, both clinically and administratively, which bring credit to the profession and serve to enhance the distinctive and contributory role of physicians.
3. Participate in multidisciplinary activities involving the treatment plan and discharge plans of patients followed by Care in the Community.
4. Utilize problem solving methodologies to resolve problems that impact patient care.
5. Review patients both clinically and administratively to determine eligibility for Community Care services.
6. Assist in the training of staff and other stake holders in the use Community Care.
7. Assist in the coordination of care with outside providers using knowledge of the resources and specialty care available in the VA.
8. Review and interpret any additional ordered tests and Requests for Services(RFS).
9. Consult review, triage, and completion.
10. Request for additional services as directed.
11. Meet indirect patient care needs through telephone calls actively managing and reviewing clinical data, communicating results to patients, communicating with other VA service departments for technical and equipment needs, and communicating with primary care providers and other referring specialists, as appropriate.
12. Foster team-oriented environment among staff to work collaboratively with administrative staff and other clinical staff within the Care in the Community; building trusting relationships.
13. Participate in and attends medical center committees as assigned.
14. Participate in various facility programs designed to enhance and improve delivery of patient care services, as assigned by Care in the Community.
15. Demonstrate commitment to high quality customer service, including specific measures identified as performance goals by the medical center.

BASIC REQUIREMENTS
Citizenship. Be a Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with [38 U.S.C. ? 7407(a)).
Education. Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine. The degree must have been obtained from an institution whose accreditation was in place for the year in which the course of study was completed. Approved schools are:
(1) Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted, or
(2) Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation of the American Osteopathic Association for the year in which the degree was granted. (3) For foreign medical graduates not covered in (1) or (2) above, facility officials must verify with the Educational Commission for Foreign Medical Graduates (ECFMG) that the applicant has met requirements for certification, and must obtain a copy of the ECFMG certificate, if claimed by the applicant. [If the applicant does not claim an ECFMG certificate, facility officials must still confirm that the medical school meets (or met) ECFMG eligibility requirements for the year the candidate graduated.
NOTE: The Under Secretary of Health or designee in the VHA Central Office may approve the appointment under authority of 38 U.S.C. ? 7405 of a physician graduate of a school of medicine not covered above if the candidate is to be assigned to a research, academic, or administrative position with no patient care responsibilities. The appointment will be made only in exceptional circumstances where the candidate's credentials clearly demonstrate high professional attainment or expertise in the specialty area.
Licensure and Registration. Physicians must possess a current, full and unrestricted license to practice medicine or surgery in a state, territory, or Commonwealth of the United States, or in the District of Columbia. The physician must maintain current registration in the state of licensure if this is a requirement for continuing active, current licensure.
Board Certification: Physicians are generally not required to be board certified for employment in VA; however, three circumstances in VA require physician board certification:
(1) If the position being filled is required to be a supervisor for medical students or physician residents (including fellows), the LCME, ACGME or AOA standards requiring a particular board certification credential will apply.
(2) If the position being filled will have faculty status with an affiliated medical school (for example, in joint recruitments with affiliated medical schools), then a medical school requirement for board certification will apply to the jointly recruited position.
(3) If the position being filled is required to be board certified by virtue of specific VHA policy (for example, as director of a cardiac catheterization laboratory or Director of Clinical Laboratory Medicine), then VHA policy requiring board certification will apply.
PHYSICIAN STAFF REQUIREMENTS
Staff Physician. None beyond the basic requirements. The individual must qualify to perform duties as a physician, have completed an accredited residency and/or possess experience which has qualified the individual to perform general duties and some specialized functions and procedures without supervision. Staff physicians may also include attendings who train physician residents assigned in facilities with residency training programs, and consultants who are capable of giving authoritative views and opinions on subjects in their field of medicine.

Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical patient lifts are provided and are to be used to preserve patient and staff safety. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements as a condition of employment.
