The Nurse Practitioner- Care In The Community (PA-CITC) will be in daily contact with the Care in the Community staff with regards to community care related activities. The NP will have intermittent contact with their supervising physician/Deputy Chief of Staff about patient care activities and for additional clinical support/guidance.

The responsibilities of the Nurse Practitioner - Care In The Community (PA-CITC) include, but not limited to the following:
1. Serves in the management of Community Care consults and requests for additional services as directed.
2. Demonstrates sound decision-making skills, both clinically and administratively, which bring credit to the profession and serve to enhance the distinctive and contributory role of physicians.
3. Participates in multidisciplinary activities involving the treatment plan and discharge plans of patients followed by Care in the Community.
4. Utilizes problem solving methodologies to resolve problems that impact patient care.
5. Ability to review patients both clinically and administratively to determine eligibility for Community Care services.
6. Assists in the training of staff and other stake holders in the use of Community Care.
7. Assists in the coordination of care with outside providers using knowledge of the resources and specialty care available in the VA.
8. Reviews and interprets any additional ordered tests and Requests for Services(RFS).
9. Consult review, triage, and completion.10. Requests for additional services as directed.
11. Meets indirect patient care needs through telephone calls actively managing and reviewing clinical data, communicating results to patients, communicating with other VA service departments for technical and equipment needs, and communicating with primary care providers and other referring specialists, as appropriate.
12. Fosters team-oriented environment among staff to work collaboratively with administrative staff and other clinical staff within the Care in the Community; building trusting relationships.
13. Participates in and attends medical center committees as assigned.
14. Participates in various facility programs designed to enhance and improve delivery of patient care services, as assigned by Care in the Community.
15. Demonstrates commitment to high quality customer service, including specific measures identified as performance goals by the medical center.
The Community Care Nurse Practitioner (NP) practices for the Sheridan VAMC as a Licensed Independent Practitioner (LIP) for the Community Care Office (CCO) providing care to Veterans.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience:
2 years of NP experience.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level III - Master's degree in nursing (MSN) and no additional nursing practice/experience.
Nurse II - Master's degree in nursing and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.
Physical Requirements:
Lifting up to 35 pounds for 8-12 hours, intermittently; moderate carrying (15-35 pounds) for 8 to 12 hours, intermittently; lifting over 35 pounds with assistive devices in accordance with the Safe Patient Handling Policy except in emergent situations requiring prompt patient care; pulling for 8 to 12 hours, intermittently; pushing for 8-12 hours, intermittently; reaching above the shoulders for 8-12 hours, intermittently, both hands required; walking 8-12 hours, intermittently; standing 8-12 hours intermittently; kneeling 8 to 12 hours, intermittently; repeated bending for 8-12 hours, intermittently; climbing and use of both legs and arms for 8-12 hours, intermittently, both legs required; near vision at 13 to 16" Jaeger 1 to 4, far vision correctable in one eye to 20/20 and to 20/40 in the other eye; ability to distinguish basic colors; hearing aid permitted; twisting,
simply grasp and fine manipulation.
The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful
situations, able to shift priorities based on patient needs. The incumbent must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment.
