The Social Worker position is in Social Work Service at the Bedford VA Healthcare Systems at the Bedford, Massachusetts location. This position is full-time at 40 hours per week.
Geriatric Care Coordination Team (GCCT) Social Worker is a part of the Geriatric Care Coordination Team (GCCT), which is envisioned under the East Market Strategic Initiative of Veteran Centric Care aiming at preventing hospitalizations and institutionalization of geriatric population.
Duties
Social Work Program Coordinator whose primary responsibility is to oversee the daily operations of GCC. GCC is a specialty program dedicated to assisting Veterans in reaching their identified goals, through time-limited community supports (i.e., education, advocacy, and collaboration) enhancing their utilization of local resources, the development of new skills and connections within their community, and to implement the treatment plan through home visits. Duties include but are not limited to:
Develops and implements GCC to their area of assignment and ensures compliance and collaboration with local facility, State, and VA guidance;
Administers information and analytical systems to evaluate and enhance the quality of service provided to Veterans, families, Caregivers, and VA employees;
Develops and implements information systems to track service activities, including visits, referrals, demographics, equipment utilization, and Veteran satisfaction;
Develops, implements, and evaluates clinical guidelines and protocols to establish appropriate utilization of services;
Participates in direct Veteran, family, Caregiver, and VA employee contact to identify and assess risk, safety, support, and the development and application of directed clinical treatment interventions;
Provides services both in the office and community off-site, including the Veteran's private home or location other than the office, as may be required, as part of providing social work case management services to Veterans and their families;
Advises and collaborates with interdisciplinary teams throughout the medical center on GCC and CC& ICM topics, creates educational tools, develops programs, and implements training focused on specific GCC needs/issues;
Develops a data monitoring system to measure the efficacy of the program;
Collects, analyzes, and reports program data to local management, Care Management, Social Work Service, and Office of Patient Care Services;
Evaluates program progress and outcomes and utilizes data to continuously modify and improve the program and its processes;
Develops policies and procedures for program operation and prepares reports and statistics for the facility, VISN, and national use;
Manages and controls the use of data related to these programs regarding day-to-day functioning and program evaluation;
Performs a wide range of services and provides technical assistance to State, local, private agencies, and universities;
Uses sound judgment in the interpretation of trends and patterns, the application of guidelines, and the recommendation of program enhancements;
Performs program analysis, development, review, research, design, and contact work with various agencies (public, DOD, State, etc.); and
Completes other assigned duties.

Work Schedule: Monday-Friday; 8:00am-4:30pm.

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in [both] spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
Education:
Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited.
A doctoral degree in social work may not be substituted for the master's degree in social work.

Grade Determinations: Social Worker (Program Coordinator), GS-12
(1) Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
(2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
(3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
(b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
(c) Ability to supervise multidisciplinary staff assigned to the program.
(d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
(e) Ability to provide training, orientation, and guidance within clinical practice.
Preferred Experience: Preferred candidates for the position wouild possess a minimun of 3 years of experience in geriatrics and data analysis /interpretation, demonstrate program develpoment and management skills, and obtain and maintain relevanrt certifications and licensure/registration.

Physical Requirements: Position requires movement throughout the clinic and/or Medical Center. Position requires travel to the community, patient homes and other sites. Demonstrates awareness of and follows all home visit safety guidelines.
