This position is in the Health Information Management (HIM) section at the Oscar G Johnson VA Medical Center. MRT's maintain, file, index and scan patient medical records and perform incomplete health record analysis. They review, analyze, abstract, maintain, extract, and compile information from the health record. MRTs HIT check for accuracy, completeness, and timeliness of the health record and monitor and report for regulatory compliance.

Duties

Major Duties: Functions Duties include the receiving, indexing, scanning and assignment of appropriate nomenclature to health and administrative information for incorporation into the correct patient's record in the computerized patient records system (CPRS). Sorts, scans, and indexes loose health record documents and materials received daily, on a timely basis and within established guidelines. Maintains and disseminates a daily productivity log of scanning and indexing activities. Performs qualitative reviews on work performed by self and other scanned documents in CPRS. Assures patient care data entered into VistA and or CPRS is accurate, timely and complete. Notifies HIMS Supervisor when images are corrupted, indexed or scanned incorrectly into patient's record. Uses and/or assists with improved processes and procedures to sort, index, and perform quality control on incoming clinical and administrative documents in order to assure ease of retrieval for users. Associates each scanned document with the correct patient's CPRS record and appropriate TIU document; assigns appropriate medical nomenclature, and electronically signs the chart. Prioritizes the scanning of Advanced Directives (AD), Power of Attorney (POA) and guardianship documents. Scans and indexes according to Medical Center policies; writes and electronically signs appropriate note. Processes requests from and transfers records to other Veterans Health Administration (VHA) medical facilities. Identifies, prepares and documents consolidated health records for disposition to the Veterans Administration Records Center and Vault. Supports peers and managers while coordinating work to meet frequently changing priorities and deadlines. The incumbent is responsible for abstraction of clinical information for ongoing regulatory reviews. Contacts physicians and other ancillary service personnel regarding dictation and/or signatures. Assists medical staff by providing information as to required documentation. Communicates very closely with the medical staff to answer questions and resolve problems to ensure quality documentation in the health records. Prepares lists of health records needed for signature(s) that are not complete in the electronic health record. Participates in approved quality improvement focused reviews. Coordinates review activities with the Quality Improvement Program to satisfy external accreditation requirements and to satisfy current Medical Center performance measures. Assists with capture and reporting of health record documentation deficiencies as needed including downloading data and creating trended reports. Performs other related duties as assigned. Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 08/16/2023. You may qualify based on your experience and/or education as described below: General Experience GS-04: Experience. One year of creditable experience that indicates knowledge of medical terminology, anatomy, physiology, and a general understanding of health records. Six months of the required one year of creditable experience must have provided the knowledge, skills, and abilities (KSAs) needed to perform MRT HIT work; OR Education: An associate degree from an accredited college or university recognized by the U.S. Department of Education, with a major field of study in health information technology/health information management, or a related degree with a minimum of 12 semester hours in health information technology/health information management (e.g., courses in medical terminology, anatomy, physiology, legal aspects of health care, and introduction to health records); (TRANSCRIPTS REQUIRED) OR Combination: Experience/Education Combination. Equivalent combinations of creditable experience and education are qualifying towards meeting basic experience requirements. The following educational/training substitutions are appropriate for combining education and creditable experience: Requirements for GS-05: Experience: One year of experience equivalent to the next lower grade level (GS-04). The employee must be able to demonstrate the knowledge and skills necessary to complete the duties of the position including those necessary to provide care appropriate to the age and population specific needs of the patients served. Such knowledge and skills are identified in the Competence Assessment Checklist of the position and include knowledge and skills in: communication and interpersonal relations including the ability to appropriately and courteously relate to internal and external customers. maintenance of confidentiality of patient/employee information, electronic and print. the medical center's Performance Improvement Program. -how to work and function in the medical center in a safe manner, including infection control and the correct operation of equipment. OR, Education. A bachelor's degree from an accredited college or university recognized by the U.S. Department of Education with a major field of study in health information management, or a related degree with a minimum of 24 semester hours in health information technology/health information management (e.g., courses in medical terminology, anatomy & physiology, legal aspects of health care, and introduction to health records). Requirement for GS-06: Experience: One year of experience equivalent to the next lower grade level (GS-05). The employee must be able to demonstrate the knowledge and skills necessary to complete the duties of the position including those necessary to provide care appropriate to the age and population specific needs of the patients served. Such knowledge and skills are identified in the Competence Assessment Checklist of the position and include knowledge and skills in: communication and interpersonal relations including the ability to appropriately and courteously relate to internal and external customers. maintenance of confidentiality of patient/employee information, electronic and print. the medical center's Performance Improvement Program. how to work and function in the medical center in a safe manner, including infection control and the correct operation of equipment. Requirements for GS-07. Experience. One year of experience equivalent to the next lower grade level (GS-06). Health Information Technician performs under the direct supervision of the Supervisor. The supervisor makes assignments by defining overall goals and objectives of the work and is available to assist the employee with unusual situations which are not covered by precedents. Continuing assignments are usually performed with considerable independence. The employee plans and carries out the successive steps and handles problems and deviations in the work assignments in accordance with instructions, policies, previous training, or accepted practices in the occupation. In more complex or unprecedented activities, the work may be subject to close technical guidance and control. Completed work is usually evaluated for technical soundness, appropriateness, and conformity to policy and requirements. You will be rated on the following Competencies for this position: Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Physical Requirements: The work with computers is mainly sedentary in the office environment. Paper records retrieval, filing, and management involve standing, bending, reaching, lifting, stooping, pull and other physical exertion. Some work requires packing boxes (whose weight will typically exceed 20 pounds) and otherwise preparing records for shipment.
