This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
Duties
This Supervisory Social Work position will provide supervision to the H-PACT social workers. Clinical supervision for licensure may also be provided to social workers at VA Puget Sound, as needed. The position will serve to promote the H-PACT program to the VA Puget Sound's internal and external partners, providing education about the program and the continuum of VA services available to Veterans experiencing homelessness, as well as VA, CHOS and H-PACT program eligibility criteria and referral processes.
Duties include, but are not limited to the following:
CLINICAL:
Responsible to provide a variety of services for Veterans assigned to the Homeless Primary Care Team (H-PACT) specialty clinic, including psychosocial assessments, treatment planning, short-term, solution focused counseling with individuals and families who are homeless and/or may be in crisis, education and referral for resources and benefits, and case management to Veterans and families throughout the continuum of care.
Provide consultation/education to Veterans and families on community resources and Veterans Administration benefits.
Use educational training, clinical experience, and knowledge to include that of VA policies and standards in administering direct services to provide psychosocial assessment of patient/family needs in relation to the provision of interdisciplinary treatment planning and service delivery, individual, group, and family counseling, and case management services for the purpose of addressing identified needs and address homelessness.
Perform visits to H-PACT Veterans who are dually enrolled in HUD-VASH at project-based facilities and/or in community settings, as well as other CHOS program settings to ensure that Veterans obtain a wide variety of services related to psychosocial issues, housing, and health care needs.
Coordinate care including serving as an advocate on behalf of Veterans to ensure comprehensive service delivery and linking and referring Veterans to the VA Medical Center, VA Regional Offices, and/or community agencies.
Conduct psychosocial assessments to determine needs of Veterans and family members and is an active participant in the treatment planning process with other disciplines and ensures appropriate hand off when transferring of patient care.
ADMINISTRATIVE:
(Supervisory)
Provide education to staff regarding whistle blower protections.
Have the ability to lead multidisciplinary staff assigned to their team. Make sustained contributions to Veteran-specific healthcare by sharing subject matter expertise within and/or outside the facility.
Provide training and supervision of a multidisciplinary team. Supervised staff may include, Social Workers, Registered Nurses, Peer Support Specialists, Social Service Assistants, Occupational Therapists, and administrative support staff. Provide administrative supervision to the team, including: Hiring, HR Actions, performance evaluations, and time and attendance procedures
Monitor compliance of staff members with VA performance measures and effectively shape provider practice patterns to improve standing on performance measures.
Lead and facilitate quality improvement projects.
Develop partnerships with community agencies and maintain regular contact and communication.

Comply with criminal conflict of interest laws and Executive Branch Standards of conduct to avoid conflicts of interest while serving as the H-PACT Social Worker.
Perform accountability functions: 1) document all contacts, interventions, and activities with patients/families in the electronic medical record and HOMES or other relevant databases; 2) complete statistical and other reports in accordance with policies and procedures.
Provide assessment functions in the service provision for Veterans experiencing homelessness in an outreach capacity, e.g. interviewing, psychosocial histories and assessments to aid in the development of treatment plans, as well as case conferencing discussions and planning with internal and external care partners.
Actively participate in conference calls, staff meetings, and attends trainings and meetings/calls offered by National, VISN, and local programs.
Maintains familiarity and responsibility to all performance measures relating to the HPACT Program.
Work with staff members within CHOS to identify Veterans and make appropriate referrals and/or warm hand-offs to internal and external housing interventions, such as HUD-VASH, Supportive Services for Veteran Families (SSVF), and other housing programs.

Qualifications
To qualify for this position, applicants must meet all requirements within 30 days of the closing date of this announcement.
BASIC REQUIREMENTS:
1. United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
2. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited.
A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
3. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
GRADE DETERMINATIONS: GS-12
1. Experience/Education. One year of experience equivalent to the GS-11 grade level.
Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
2. Licensure/Certification. Individuals assigned as Supervisory Social Workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
3. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs:
(a) Ability to independently organize work, set priorities, and meet multiple deadlines.
(b) Knowledge of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement.
(c) Ability to ensure provision of clinical social work services.
(d) Ability to provide staff training and development.
(e) Skill in interpersonal relationships in dealing with employees, team leaders, and managers.

The full performance level of this vacancy is GS-12.
PHYSICAL REQUIREMENTS: Position requires moderate lifting & carrying (15-44 pounds), pushing (6 hours), operation of motor vehicle, ability to distinguish basic colors, and hearing (aid may be permitted).
