The VA St. Louis Healthcare System is looking for a Clinical Pharmacy Specialist who will focus in telehealth services. This position is responsible for initiating, modifying, or discontinuing medication therapy, consulting for intensive medication therapy management services, fiscally responsible medication use, providing drug information services and patient/family education, and developing and managing pharmacy programs related to the practice area.

Duties

Clinical Responsibilities Complex medication therapy management: initiate, modify, and monitor pharmaceutical care plans pertaining to medical conditions managed. Assess the appropriateness and effectiveness of patients' medication regimens by reviewing medical records, laboratory values, medication monitoring parameters, other interdisciplinary communications and interview/examining the patients as appropriate. Serve as a consultant to other providers for intensive medication therapy management services Documentation in the medical record of all relevant findings from drug therapy monitoring and management activities in a timely manner for other healthcare professionals who are responsible for patient care. Provides patient care that is appropriate to the cognitive, emotional, and chronological maturation needs of the adult and/or geriatric patient. Educational Responsibilities Conducts in-service education programs for physicians, nursing, pharmacy staff and others as assigned. Educates and trains other health care disciplines on the safe, rational, and cost-effective use of medications. Maintaining and improving professional competency by keeping up with information on new drugs, new drug therapies, and innovations in pharmacy practice. Participates in training of pharmacy residents and students. Administrative and Organizational Support Assist Pharmacy and Patient Care Service in accomplishing the facility's mission and goals as related to the provision of pharmaceutical care. Actively manage patient care clinics, schedule, and workload. Contribute to medical center committees when assigned to provide thoroughly investigated, unbiased, well-documented drug information. Participate in scholarly and QM activities to improve safe and effective medication use and clinical pharmacy functions. Intervention reporting: Documents/collects patient outcomes data on a periodic basis as required. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification. Grade Determinations: To be considered at the GS-13 grade you must meet the basic requirements and the requirements listed below: Experience: You must have 1 year of specialized experience equivalent to the next lower grade level, GS-12. You must possess the following knowledge, skills and abilities, (KSAs): Knowledge of professional pharmacy practice. Ability to communicate orally and in writing to both patients and health care staff. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in the program area. Expert knowledge of specialized program area. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Completion of an ASHP accredited PGY-1 Residency program. Preferred qualifications are at least 1 year clinical, direct patient care experience beyond PGY1 training within the last 3 years. Experience with centralized, high volume patient care clinics. Other preferred credentials include Board of Pharmaceutical Specialties certification, CDECS, or BC-ADM certification.
