The Registered Nurse (RN) Program Manager (PM) for the Community Living Center (CLC) works in close collaboration with the Chief Nurse (CN) CLC guiding and directing the operations of the CLC to ensure administrative requirements/functions of the CLC including compliance with Federal Tags for Centers Medicaid/Medicare Services (CMS) and al! other accrediting/regulatory agencies are implemented.
Duties
The RN/PM must ensure the care and services provided meet the satisfaction of the residents and their families, thus maintaining/increasing customer satisfaction. He/she serves as primary liaison with residents, their families and the general public. He/she serves as facilitator of the CLC Resident Council and Family Council Meetings. He/she works in close collaboration with the VACHS Patient Representative Office, CLC Nurse Managers and Interdisciplinary.
Team Members to assure all complaints/concerns regarding the CLC are resolved promptly and satisfactory. He/she aggregates and analyzes all data regarding CLC complaints/concerns to observe for patterns/trends and design/implement recommendations for improvement.
The RN/PM CLC utilizes the Holistic Approach to Transformational Change (HATCh) model of Culture Transformation. He/she works with the six inter-related domains that lead to individual, organizational, community and systems changes which are Workplace Practice, Care Practice, Environment, Leadership, Family and Community and Regulatory/Government. In addition, he/she serves as liaison with the Recreational Therapist to ensure compliance with Federal Tags for CMS.
He/she serves as liaison with Facility Management Service and Safety to ensure that the CLC areas comply with all Environment of Care (EOC) and K-Tags standards and safety regulations. He/she performs recurrent rounds, random spot checks and tracers to ensure compliance with F-Tags CMS environmental standards and provides support to ensure work orders are completed. He/she also follows up on CLC renovations and minor projects.
He/she works in close collaboration with Logistics Service in the purchase of equipment as it entails nursing expertise to:
Evaluate equipment available
Justify equipment needed
Perform technical reviews of equipment to ensure the vendor selected has the best equipment to meet the Veteran's and staffing needs
Evaluate Reusable Medical Equipment to determine if we have the capacity to meet the manufactures guidelines
He/she ensures compliance with the Electronic Inventory List (EL). He/she is responsible for developing and overseeing the budget for the CLC Fund Control Point (FCP). He/she sets up controls to monitor expenses during the year and recommends budget adjustments including restructuring budget allocations or work plans to deal with changing situations such as varying costs for services and changes in the availability of funds. The RN/PM CLC functions as Alternate Approving Official (AO) for the CLC purchase card holder.
He/she functions as CLC Admission Screening Coordinator and as such carries out the following functions:
Receiving the consult to assure that all requisites are available and within established times frame including the medical laboratory, medical test, LTC co-payment, referring social work assessment, the 'Wandering Risk Assessment" and discharge planning.
If any requisites are missing he/she will receive and complete the consult indicating what is missing and requesting the referring source to re-consult when requisites are available.
Ensuring the CLC Medical Director/designee determines the medical status, level of treatment, prognosis of candidate and the candidates' medical and psychiatric stability.
Assuring CLC staff has the competencies to provide the service the Veteran requires.
Assuring staff receive the proper training and competency validation prior to accepting a Veteran that needs a service not commonly practiced in the CLC but currently performed in homes.
Assuring Veteran's medical and/or psychiatric conditions can be managed in the CLC by the nursing staff.
Discussing the LTC Admission Agreement with Veteran/next of kin.
Closing the CLC consult with the team's final determination and if accepted assuring the CLC Medical Director/designee writes the admission orders in delay orders.
Maintain open communication with the referring provider and social worker.
Assigning CLC bed as per Veteran's needs.
He/she is actively involved in Quality Assurance Performance Improvement (QAPJ) and in staff education. He/she stays abreast of health care trends in service and financing.

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 07/21/2023.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. Must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Must be proficient in written and spoken English
Preferred Experience:
Must possess a minimum of five years of nursing practice / experience must possess at least two (2) years performed in a long term care setting, preferred.
Knowledge of Resident Assessment Instrument Minimum Data Set (RAI-MDS), preferred.
Knowledge of Center of Medicare and Medicaid Services (CMS) federal regulations and guidelines, preferred.
Knowledge in skilled nursing care, rehabilitation, restorative care, long term spinal cord injury diseases, hospice and palliative care, preferred.
Knowledge of Skin and Wound Care programs, Fall Prevention programs, Restorative care programs, Nutrition programs and Self-medication programs, preferred.
Spanish proficiency preferred.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.

Physical Requirements: This position involves lifting, moving and transporting patients, assist with transferring patients from wheelchair to to stretcher or radiology suite table and vice versa. Also assist with transferring patients to wheelchairs and/or may need to escort a patient to other areas. This position involves exposure to blood borne pathogens, fine manipulation, standing, walking, bending, crouching and reaching patients. It requires dealing with the acutely ill, with patients who may display disturbed behaviors, and with stressful situations. Moving patients and weighing patients in excess of 50 kg. or more. Wearing of a respirator may be required at times in the performance of the duties of this position. When required to use respirator, employee cannot have facial hair or any condition that interferes with the face to face piece seal or valve function of the respirator.
