The Harry S. Truman Veterans' Hospital in Columbia, Missouri, is recruiting for a Pharmacist (Clinical Pharmacy Specialist/Practitioner) - Geriatric Patient Aligned Care Team (Geri-PACT). This position reports to the Associate Chief of Pharmacy for Clinical Programs.

VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws The Clinical Pharmacist Practitioner (CPP) functions at the highest level of clinical practice, working independently under an advanced scope of practice greater than 25% of their time. The CPP functions as a member of assigned team(s) and serves as an advanced practice provider to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing patient assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Administrative Participates in Professional Practice Evaluation Activities required to maintain an Advanced Scope of Practice. Performs other administrative duties as assigned by the Chief of Pharmacy Service, Associate Chief for Clinical Pharmacy Services and the Chief Medical Officer. May be required to perform administrative duties necessary for performance measures or improvement activities, compliance with accrediting organizations, or other duties as assigned. Clinical Assumes clinical responsibility for the Geri-PACT, Geriatric Evaluation Clinic (GEC), and Caring for Older Adults and their Caregivers at Home (COACH) areas of practice. This will include maintaining awareness and understanding of ongoing Pharmacy programs, as well as assuring competency to perform under an approved Scope of Practice. Core functions include face-to-face, telephone and telehealth encounters with referred patients, for comprehensive medication management in practice areas identified in the individual scope of practice. Accesses appropriate patient records for proper evaluations of clinical situations and therapeutic decision making. Conducts patient interviews to determine a medication history and additional information regarding disease state/drug therapy when evaluating a clinical situation. When necessary, educates patients on the proper use and expected effects of their medications. Develops individualized therapeutic plans utilizing the most efficacious, least toxic, and most economical pharmacological treatment modalities available, and communicates interventions verbally or in writing to pertinent members of the healthcare team. Monitors and assesses therapeutic and adverse effects of drug therapy (including drug interactions) through selection and evaluation of physical and laboratory parameters. Documents pharmaceutical care plans in a timely and accurate manner. Participates in population management initiatives to optimize therapeutic outcomes, facility performance metrics and CPP resources. Participates in the investigation, documentation, and reporting of adverse drug reactions and medication errors. Provides drug information to the Medical Center staff concerning proper drug selection, optimal dosage and route of administration, recognition and management of drug interactions and reactions, and proper handling and storage of medications. Serves as an authoritative information source on drugs and their utilization in therapy, with special emphasis on medications used in treatment of the assigned patients. Education and Training A. Serves as a preceptor to students of Affiliated Schools of Pharmacy. Serves as a preceptor to pharmacy residents. Assists in the training of new pharmacy staff. Designs, implements and supervises the clinical education and training for pharmacists at the Truman VA. Provides highly specialized education and training to other professional services in the area of advanced pharmacotherapeutics. Performs in-service training as scheduled. Research Independently secures funding and conducts clinical research within the scope of Geriatric Care/Primary Care. Actively collaborates in the design and implementation of research protocols, collection of data and participates in the analysis and interpretation of results. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. OR, Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). All licensed pharmacists employed in VHA in this occupation on the effective date of June 7, 2012 are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. Grade Determinations: GS-13: Experience or Education: One year of experience equivalent to the next lower grade level (GS-12). AND Applicant must demonstrate the following knowledge, skills and abilities (KSAs): Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Successful completion of a PGY-1 and PGY-2 Geriatric Pharmacy Residency BCPS/BCGP Certification References: VA Handbook 5005, Part II, Appendix G15. The full performance level of this vacancy is GS-13. The actual grade at which an applicant may be selected for this vacancy is GS-13. Physical Requirements: Moderate lifting (15-44 pounds), Light carrying (under 15 pounds),Straight pulling (8 hours), Pulling hand over hand (8 hours), Reaching above shoulder, Use of fingers with both hands, Far vision correctable in one eye 20/20and to 20/40 in the other, Ability to distinguish basic colors, Mental and Emotional Stability required.
