This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. The Clinical Pharmacy Practitioner (CPP), Home Based Primary Care, utilizes expertise in the area of Home Based Primary Care to provide medication management to optimize patient care.
As an advanced practitioner of Home Based Primary Care he/she is authorized within the guidelines of a scope of practice to function as a provider of services to patients of the VA TVHS requiring minimal supervision in both the practice area of Home Based Primary Care as well as the specialty practice area in which he/she is assigned.
The CPP Home Based Primary Care utilizes specialized expertise in the area of Home Based Primary Care aligning with the goals and objectives of medicine and specialty services to optimize patient care and is responsible for oversight of all clinical pharmacy activities and consultations in Home Based Primary Care settings for patients in whom they are assigned.
Principal responsibilities include evidence-based pharmacotherapy management (including identification of patient specific medication issues such as drug interactions, adverse effects, lack of effectiveness, appropriateness, and compliance problems), and provision of drug information service to medicine and specialty services both inpatient/outpatient and to other areas within Clinical Pharmacy Service as requested.
Attends and participates in team meetings, conferences, and other meetings as required for the purpose of making recommendations and providing consultation in all Pharmacy-related matters to Home Based Primary Care for the Pharmacy service line.
Directly assists other providers in rational medication selection by providing written and/or verbal consultation.
Is responsible for planning, developing, and maintaining effective clinical pharmacy programs by implementing rational cost-effective drug therapy to improve patient outcomes within medicine and specialty services.
Provides medication information to medicine and specialty team members as required.
Provides assistance in development of policies as requested.
Participates in developing criteria for drug utilization evaluations, data collection, trending reports, and other quality assurance activities as required to ensure appropriate and safe drug utilization.
Assists in initiation of medication use improvement strategies including development of quick order sets, clinical reminders, patient/provider education handouts, and development and implementation of pharmacotherapy protocols.
Ensures the safety and efficacy of the medication use process in the area assigned by reporting adverse drug effects, medication errors, and other quality issues.
Performs data analysis to measure performance, trend changes, and implement quality improvement strategies.
Serves as liaison to pharmacy service for Home Based Primary Care services, serving as the local VAMC subject matter expert in Home Based Primary Care.
Reports directly to and requires minimal supervision by the Primary Care/Outpatient Clinical Pharmacy Programs Supervisor.
The incumbent will ensure that all prescriptions and actions meet proper eligibility criteria on a patient specific basis and the National Formulary and Non-Formulary procedures are followed. This includes adherence to all national, VISN and local criteria or use treatment algorithms.
Assures compliance with VA and other accrediting bodies in terms of meeting all medication use standards.
Participates in the appropriate, effective, and safe use of pharmaceuticals by contributing to criteria development, data collection and drug use evaluation and other quality improvement activities as assigned.
Determines if a referral to a physician is necessary when disease progression occurs, or adverse drug events require a higher level of treatment intervention.
Accepts urgent or same day patient visits including, but not limited to, patient medication review for polypharmacy, recent hospital or emergency department discharges, and co-managed care patients.
Follows up via face-to-face, telehealth, eConsult, telephone, secure messaging, and other modalities to provide patient care.
This position will provide >51% Direct Patient Care.
Maintains current knowledge in Home Based Primary Care in accordance with published guidelines and generally recognized standards of care.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.
English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
Preferred Experience: Doctor of Pharmacy degree plus (1), (2), or (3), below: 1) Completion of an ASHP accredited PGY-1 Pharmacy Practice residency and Three (3) years of practice with substantial component of patient care activities acting as a mid-level provider in the specialty area of home based primary care. 2) Completion BOTH an ASHP accredited PGY-1 Pharmacy Practice and a PGY-2 Ambulatory Care Residency with focus in chronic disease state management and three additional years of practice with a substantial component of patient care activities serving as a mid-level provider in specialty area of home based primary care. 3) Completion of BOTH an ASHP accredited PGY-1 Pharmacy Practice Residency and a AHSP PGY-2 Specialty Ambulatory Care Residency with a substantial component of patient care activities during the residency, serving as a mid-level provider in specialty area of home based primary care.
Grade Determinations:
a) Experience. This position is above the full performance level for Licensed Pharmacists. 1-year of experience equivalent to the GS-12 level is required. This experience would allow you competently answer the following GS-12 level KSAs: 1. Knowledge of professional pharmacy practice. 2. Ability to communicate orally and in writing to both patients and health care staff. 3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. 4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters
.b.) Assignments: For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time.
Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution andserves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following KSAs:
Ability to communicate orally and in writing to persuade and influence clinical and management decisions
Expert understanding of regulatory and quality standards for their program area
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters

The full performance level of this vacancy is GS-13.
Physical Requirements: Applicants must be able to perform the following duties: lifting (up to 50 lbs); carrying (up to 50 pounds). Applicants must have use of all fingers. Walking or standing for up to 8-hours. Kneeling intermittently multiple times for up to 8-hours. Use of both legs is required. Must possess visual acuity to read typed print.
